Case Manager (AMH-TCM)

Position Hours: 40 hours per week

Position Schedule: Monday – Friday, 8:00 a.m. – 4:30 p.m. (flexible)

Reports To: Director of Case Management

Salary: $24.04 to $28.85 per hour

FLSA: Hourly

Job Summary:

At Affinity Care MN, our AMH-TCM Case Managers play a vital role in supporting adults living with Serious and Persistent Mental Illness (SPMI). You will partner with individuals to navigate the mental health system, facilitate access to medically necessary services, and promote recovery-oriented outcomes. This role requires a clinical lens to assess functional impairments, monitor psychiatric stability, and provide crisis intervention support. This is a community-based role requiring frequent in-home and community visits. It requires strong clinical judgment, crisis de-escalation skills, and meticulous documentation.

Key Responsibilities:

  • Will have a caseload of around 26 - 30 clients.
  • Conduct comprehensive functional assessments to determine what needs the client may have.
  • Develop person-centered Individual Community Support Plans (ICSP) that address functional impairments and recovery goals.
  • Collaborate with multidisciplinary treatment teams, including psychiatrists, therapists, and crisis responders, to ensure integrated care.
  • Identify and connect individuals with providers who can meet their assessed needs.
  • Maintain ongoing communication with individuals, families, and providers to address evolving needs and psychiatric stability.
  • Complete face-to-face visits in the individual’s home or another safe community location at least monthly (or as required by county/state mandates).
  • Ensure all services meet the individual’s preferences and comply with county, state, and federal policies.
  • Provide timely crisis intervention, including safety planning and coordination with emergency services when necessary.
  • Advocate for each individual’s needs, goals, and independence—including their right to make informed risks.
  • Maintain the "Golden Thread" of documentation: ensuring that Diagnostic Assessments, ICSPs, and progress notes are clinically linked and compliant with state and county standards.
  • Stay current with updates or changes from DHS, counties, and other governing agencies.

Environment & Expectations:

  • This position requires a balance between community-based fieldwork and office-based administrative tasks.
  • Office Location: This role is based in our Bloomington, MN, office. Staff are expected to be on-site or in the community as required by caseload needs.
  • Fieldwork: Case Managers are expected to be mobile, conducting frequent in-person visits in clients' homes and other community settings across the Twin Cities metro area.
  • Remote Work Policy: This is not a remote-work position. Working from home is permitted only for incidental purposes (e.g., completing documentation when office access is unavailable) and must be approved in advance by the Director.
Requirements:
  • Education: Bachelor’s degree in social work, psychology, nursing, human services, or a related field; plus one year of experience working with individuals with a mental health diagnosis.
  • Understanding of the Minnesota Department of Human Services (DHS) mental health statutes and Targeted Case Management (TCM) documentation requirements.
  • Ability to identify symptoms of mental illness, recognize signs of crisis/decompensation, and utilize trauma-informed care principles.
  • Strong written and verbal communication skills; ability to document clinical observations accurately.
  • Ability to maintain professional boundaries, work independently, and navigate change with flexibility.
  • Intermediate computer skills and proficiency in using an EHR (electronic health record) system.
  • Valid driver’s license, reliable transportation, and auto insurance (travel in the Twin Cities and occasional travel outside metro required).
  • Just travel to and from clients; no client transportation required.
  • Ability to pass a DHS background study.
