Salary Range: $103,525 - $121,794
Project HOME is seeking a Family Nurse Practitioner to join our integrated healthcare team and help eliminate health disparities for individuals and families experiencing homelessness, housing insecurity, and systemic barriers to care. As a Family Nurse Practitioner, you will deliver primary care services to children, adults, and OB/GYN patients.
The hours are Monday-Friday, 8:00 AM-4:00 PM located at the Stephen Klein Wellness Center — 2144 Cecil B. Moore Ave, Philadelphia, PA 19121.
What You’ll Do:
As a key member of our community-based care team, you’ll:
- See 9 patients per session, 8 sessions per week, with a focus on holistic and trauma-informed care
- Collaborate with a dedicated team of medical assistants, behavioral health consultants, care managers, and peer specialists
- Address both clinical and social determinants of health, recognizing the unique needs of marginalized populations
- Uphold our mission and values through ethical, equitable, and evidence-based care
- Participate in quality improvement initiatives, staff development, and clinical leadership opportunities
- Mentor students (NP, PA, and MD) and engage in ongoing CME for professional growth

What We’re Looking For:
- Master’s Degree and current NP license (Family Nurse Practitioner required)
- With training or experience in gynecology, obstetrics, and pediatrics
- CPR and DEA certification
- Experience or strong interest in serving underserved and vulnerable populations
- Comfort working with patients experiencing behavioral health issues, trauma, or housing insecurity
- Strong communication, clinical, and organizational skills
- Experience with eClinicalWorks or similar EHR systems a plus
