Summary of Responsibilities:
The Nurse Resident Care Director (RCD) oversees all resident care services to ensure high-quality, person-centered care in accordance with applicable federal, state, and local regulations.
The Resident Care Director serves as the designated clinical authority within the community, responsible for oversight of resident care services under the direction of the Executive Director. This role partners with the Executive Director to ensure compliance, operational efficiency, and quality outcomes.
This position is responsible for ensuring resident care services comply with all applicable federal, state, and local regulations governing personal care homes and related healthcare services.
Essential Functions/ Duties:
Essential duties may include, but are not limited to:
Leadership & Supervision
- Supervise, coach, and evaluate Resident Care Coordinator, Medication Technicians, Resident Assistants, and other direct care staff
- Ensure staff adherence to care standards, documentation requirements, and company policies
- Provide regular on-the-floor leadership, guidance, and support
- Participate in and support hiring, onboarding, training, and disciplinary actions in collaboration with the Executive Director and Human Resources
- Foster a positive, team-oriented work environment focused on accountability and quality care
Regulatory Compliance & Clinical Oversight
- Ensure resident care services comply with all applicable federal, state, and local regulations governing personal care homes
- Maintain survey readiness and participate in inspections and audits
- Oversee completion and accuracy of resident assessments, support plans, MARs, and incident reports
- Ensure proper medication management, including storage, administration oversight, and documentation
Resident Assessment & Care Planning
- Conduct pre-admission assessments to determine appropriateness for admission
- Complete required assessments and reassessments (including semi-annual and change-of-condition)
- Develop and update individualized support plans in collaboration with the care team
- Monitor resident conditions and identify changes requiring intervention or higher levels of care
- Notify the Executive Director and appropriate parties of significant changes in resident needs
Resident Care Coordination
- Coordinate care with physicians, hospice providers, pharmacies, and other external partners
- Facilitate communication during transitions of care, including hospitals and skilled nursing facilities
- Ensure timely and accurate documentation for admissions, discharges, and transfers
- Lead or participate in care plan meetings with residents, families, and care teams
- Delegate operational coordination tasks to the Resident Care Coordinator as appropriate
Staffing & Operations
- Oversee staffing within the resident care department, including staffing levels, coverage, and performance, and delegate scheduling coordination as appropriate
- Monitor staffing levels, attendance, and performance concerns
- Ensure medication carts, treatment areas, and care documentation meet regulatory and company standards
Quality Assurance & Risk Management
- Monitor quality of care and implement improvements as needed
- Review incidents and ensure appropriate follow-up and documentation
- Ensure adherence to infection control and safety standards
- Identify risks and implement corrective actions to maintain a safe environment
On-Call & Emergency Response
- Participate in on-call rotation as assigned
- Respond to clinical and operational emergencies
- Provide direction and support to staff during urgent situations
Supervisory Responsibilities:
- Direct supervision of Resident Care Coordinator, Medication Technicians, Resident Assistants, Certified Nursing Assistants, and other direct care staff.
Position Type/ Expected Hours of Work:
- Full-time, exempt position
- Schedule may include evenings, weekends, holidays, and on-call responsibilities
- Availability required to support staffing and resident care needs
Required Skills/ Abilities:
- Strong knowledge of geriatric care practices and resident-centered care
- Understanding of dementia care and behavioral management techniques
- Knowledge of applicable federal, state, and local regulations governing personal care homes
- Strong leadership, coaching, and team development skills
- Excellent critical thinking and problem-solving abilities
- Effective communication and interpersonal skills
- Ability to manage multiple priorities in a fast-paced environment
- Proficiency in electronic health records, medication systems, and Microsoft Office
Education and Experience Requirements:
- High school diploma or equivalent. (Required)
- Graduate of an accredited Licensed Practical Nurse (LPN) program (Required)
- Current, active LPN license in the applicable state of employment (Required)
- Associate degree or higher in nursing or related field (Preferred)
- Minimum of three (3) years of experience in senior living, personal care, or healthcare setting (Required)
- Prior supervisory or leadership experience (Strongly Preferred)
- Experience with dementia care and personal care home operations (Preferred)
Work Authorization/ Clearances:
- Criminal History Clearances
- FBI fingerprint clearance (if required)
- Pre-employment drug screening (if applicable)
- Proof of eligibility to work in the United States
Physical Requirements and Physical Demands:
- Ability to stand, walk, and move throughout the community for extended periods
- Ability to assist with resident care tasks when necessary
- Ability to lift, transfer, or assist residents in accordance with safety guidelines
- Ability to respond quickly in emergency situations
- Ability to operate standard office and healthcare equipment
