Job Description
Pine Park Health is a value-based primary care practice that is revolutionizing healthcare for residents of senior living communities. We empower seniors to get healthy, stay healthy, and lead lives they love through a care model designed with everyone in mind — patients, families, community staff, providers, and payers.
Our approach brings healthcare directly to seniors' homes, offering prevention, screening, chronic condition management, lab work, and diagnostic testing in the comfort of their apartments. With weekly community visits, our teams provide consistent care while collaborating closely with facility staff to address comprehensive health needs. We've eliminated unnecessary barriers to urgent care through same/next-day appointments, helping seniors avoid emergency rooms and hospitalizations where they risk exposure to additional health concerns. Today, over 185+ communities across Arizona, California, and Nevada trust Pine Park Health, and we're rapidly expanding our reach and impact. If you're mission-driven and passionate about transforming senior healthcare, this is your opportunity to make a meaningful difference!
The Opportunity
Are you an experienced Nurse Practitioner who still loves patient care — and is ready for broader impact through leadership?
Pine Park Health is hiring a Medical Director, Nurse Practitioner to lead and develop our Bay Area clinical team while staying connected to practice. You’ll coach a small team of NPs and PAs, drive provider performance and clinical quality, help hire and grow the team, and continue caring for older adults in senior living communities.
How This Role Works
This is a Bay Area clinical leadership role that balances people leadership and patient care. You’ll spend time on-site with your team and community partners as needed, with flexibility to plan that time around what your providers and patients need — plus mileage reimbursement and Friday WFH learning days so it stays sustainable.
You do not need a Director title today. Lead NPs, APP Leads, NP/APP Managers, Clinical Managers, Clinical Supervisors, and similar leaders are strongly encouraged to apply.
Compensation & Benefits
- Total target cash: $200,000-$240,000 (base + bonus included)
- Sign-on: Eligible for a sign-on bonus of up to $15,000 based on experience and overall offer package
- Equity and comprehensive benefits
- PTO: Up to 6+ weeks annually (vacation, sick, and paid holidays)
- Friday work-from-home
- Professional development funding
- Medical, dental, and vision
- 401(k)
- Company-issued cell phone
- Mileage reimbursement
- Paid UpToDate subscription
Final compensation reflects clinical experience and depth of clinical and people-leadership experience.
What You’ll Do

- Lead, coach, and develop a small team of NPs and PAs across the Bay Area
- Spend regular time in the field alongside providers — observe care, coach in real time, and stay close to day-to-day work
- Hold 1:1s; give ongoing feedback and professional development
- Coach providers toward strong clinical and operational performance, using meaningful measures such as quality, documentation, access, and care-gap closure.
- Help shape provider schedules, patient panels, and coverage models to support both excellent care and a sustainable clinician experience.
- Hire, onboard, train, and develop new clinicians
- Lead quality improvement and strengthen clinical workflows
- Partner with clinical, operations, growth, and other cross-functional leaders
Build community partnerships
- Build relationships with senior living community leaders and staff
- Serve as a clinical leadership point of contact for partners
- Support new community launches and growth of existing partnerships
- Partner with community teams to address clinical and operational challenges while keeping relationships strong
Stay connected to patient care
- Provide primary care to older adults in senior living communities
- Deliver Welcome Visits, Care Plans, AWVs, assessments, medication management, and follow-up care
- Support post-discharge transitions from acute or SNF settings
- Respond to urgent medical and behavioral health needs
- Provide coverage when teammates are out
- Help prevent avoidable ED visits and hospitalizations
Build your leadership career
This role is designed for an experienced NP who is ready for the next step in clinical leadership. You'll gain hands-on experience in provider development, hiring, performance management, quality improvement, and market-level clinical operations while remaining connected to patient care.
As Pine Park Health grows, you'll have the opportunity to expand your leadership scope and influence how we deliver care across the market.
