Job Description
We are seeking a Psychiatric Mental Health Nurse Practitioner (PMHNP) to join our healthcare facility. Psychiatric Mental Health Providers work with individuals and families to assess, diagnose, treat, and manage mental health conditions across the lifespan. The PMHNP provides psychiatric evaluations, medication management, treatment planning, patient education, and ongoing mental health care while collaborating with other members of the healthcare team to improve patient outcomes.
Primary Responsibilities- Conduct comprehensive psychiatric evaluations for new and established patients.
- Assess, diagnose, and treat mental health and behavioral health disorders utilizing evidence-based practices.
- Develop individualized treatment plans based on patient needs, diagnoses, goals, and clinical presentation.
- Provide medication management services, including prescribing, monitoring, and adjusting psychotropic medications as clinically appropriate.
- Monitor patient progress and treatment effectiveness through routine follow-up appointments and ongoing assessments.
- Utilize validated screening and assessment tools to assist with diagnosis, treatment planning, and therapeutic monitoring.
- Educate patients and families regarding diagnoses, medications, treatment options, side effects, and expected outcomes.
- Provide supportive counseling and therapeutic interventions within the scope of practice.
- Coordinate care with therapists, primary care providers, psychiatrists, and other healthcare professionals to ensure continuity of care.
- Refer patients to higher levels of care, specialty services, or community resources when clinically indicated.
- Maintain accurate, timely, and complete documentation in accordance with organizational policies, professional standards, and applicable regulatory requirements.
- Communicate with patients regarding treatment recommendations, laboratory results, follow-up care, and other pertinent clinical information.
- Participate as a member of an integrated, multidisciplinary treatment team.
- Ensure compliance with all applicable state and federal laws, regulations, professional standards, and ethical guidelines governing psychiatric practice.
- Participate in quality improvement initiatives and organizational efforts to enhance patient outcomes and service delivery.
- Promote patient engagement, treatment adherence, and recovery-oriented care.
- Prepare and maintain documentation, records, and reports in accordance with the policies and procedures of Freeman Health Partners Medical Services, PC.
- Psychiatric Mental Health Nurse Practitioner (PMHNP), unrestricted license in the State of Tennessee.
- Advanced Practice Registered Nurse (APRN), licensed in the State of Tennessee.
- Registered Nurse, licensed in the State of Tennessee.
- Board Certification as a Psychiatric Mental Health Nurse Practitioner.
- Master of Science in Nursing (MSN) or Doctor of Nursing Practice (DNP) from an accredited program.
- Current DEA registration and eligibility to prescribe controlled substances in Tennessee.
- CPR certification.
- Excellent clinical assessment, diagnostic, and treatment planning skills.
- Ability to work independently while collaborating effectively within a multidisciplinary team.
- Excellent communication and interpersonal skills.
- Ability to establish empathy and provide person-centered, trauma-informed care.
- Strong commitment to ethical practice, patient advocacy, and quality clinical outcomes.
- Have a strong dedication to a wellness lifestyle.
This position requires a ninety (90) day written notice in the event that the employee is terminating their position.
