Job Description
We are seeking a Skilled Nursing Facility (SNF) Nurse Practitioner to diagnose and treat health conditions and promote optimal health for residents in partnered Skilled Nursing Facilities. As an advanced practice provider, you will ensure high-quality, efficient, and compassionate patient care within the SNF environment.
Responsibilities include conducting exams, interpreting diagnostics, managing acute and chronic illness, and collaborating closely with nursing staff to enhance patient outcomes. Experience in primary care, geriatrics, SNFs/LTC, hospital medicine, or mobile medicine is preferred.

This position offers autonomy, flexibility, and the opportunity to work in a mobile healthcare environment, providing care to patients at Alta Heights in Goodlettsville, TN.
Responsibilities:
- Clinical Care
- Examine SNF residents and review their medical records
- Order and interpret diagnostic tests (labs, imaging, etc.)
- Diagnose, manage, and treat acute and chronic health conditions
- Provide oversight for residents’ overall medical status
- Propose and monitor treatment plans for chronic and infectious diseases
- Prescribe medications in accordance with state and federal regulations
- Manage transitions of care and participate in change-of-condition evaluations
- Educate residents and their families regarding diagnoses, treatment plans, and preventive health
- Promote disease prevention, wellness strategies, and safe medication practices
- Support wound care, behavioral health screening, and chronic disease optimization
- Coordination & Documentation
- Maintain accurate, timely documentation in the electronic health record
- Collaborate closely with SNF nurses, administrators, therapists, and attending physicians
- Participate in interdisciplinary care meetings as required
- Communicate changes-of-condition promptly to facility staff and IMG leadership
- Uphold compliance with CMS, state SNF regulations, and internal policies
- Assist in reducing avoidable hospitalizations through proactive clinical interventions

Skills & Qualifications
- Proven experience as an NP or PA in primary care, SNF/LTC, hospital, ER, or similar setting
- Strong clinical skills in acute care, chronic disease management, and geriatric medicine
- Knowledge of healthcare and post-acute care regulations
- Excellent communication, interpersonal, and documentation skills
- Strong clinical judgment and problem-solving ability
- Positive attitude and team-oriented mindset
- Commitment to lifelong learning and clinical development
- Valid Nurse Practitioner or Physician Assistant license (TN)
- National board certification (AANP, ANCC, NCCPA)
- DEA license or ability to obtain
