RESPONSIBILITIES:

Provide information, referrals and wrap around services to participants in the Comprehensive Case Management and Employment Program (CCMEP). CCMEP participants are referred from the Mahoning and Columbiana Training Association and are working toward self-sufficiency by preparing for employment.
Interview participants during an intake to assess and identify problems and then assist with helping to overcome the problems to prepare them to start the CCMEP employment process.
Establish, nurture and maintain a mentoring relationship with participant to develop trust and ensure success with the CCMEP employment process.
Collaborate with MCTA referral source and update on participant progress.
Attend meetings when necessary and arrange for participant team meeting.
Maintain appropriate linkages to internal and external social service resources.
Link participants to emergency assistance and crisis intervention, when necessary, and collaborate with community resources such as local governments, non-profits, childcare centers, and private agencies to identify services and programs available to assist participants.
Update electronic health record files by recording each contact made with participant and maintain records required for follow-up.
Maintain confidentiality in accordance with all applicable laws.
Other duties as assigned.

QUALIFICATIONS:

Bachelor’s degree in social services or human services field, or experience equivalent. Case Management experience a plus.
workforce development experience preferred.
Knowledge of labor market information and vocational training tools a plus
Must be flexible and willing to adapt to new methods and responsibilities that accompany any new grant opportunities.
Knowledge of community employment resources and job market, ability to work independently, excellent oral and written communication skills, strong organizational skills and flexibility.
Positive attitude toward clients and public, responsive to questions or concerns raised by others, willingness to work a flexible schedule, adaptable, creative, and initiating.
Knowledge of community and other resource agencies.

PHYSICAL REQUIREMENTS:

Prolonged periods of standing and walking.
Frequently required to sit, reach with hands and arms, and stoop, kneel, crouch, or crawl.
Must be able to lift and/or move 25 pounds.
