Position Summary
Provides clinical guidance, support and development for acute care and ambulatory social work and palliative care services. Responsible for oversight of all social work practices delivered both inpatient and outpatient across the organization. Provides leadership in the design and implementation of processes designed to address complex discharges and coordination among inpatient, outpatient and post-acute providers
Responsibilities
• Ensures compliance with regulatory and licensing requirements
• Establishes performance metrics, monitors progress, addresses opportunities for improvement and develops core competencies among staff
• Guides department operations according to the organizational objectives, hospital policies, and standards of practice; Federal and State Regulations and accreditation guidelines
• Maintains knowledge and expertise in best practices in the field of social work and palliative care
• Provides vision and direction for case management across the acute care continuum, integrating social work, discharge planning, and resource utilization into coordinated workflows
• Oversees completion of trauma related assessments for patients presenting with psychological distress, injury related stress responses, or suspected PTSD, SBIRT processes for alcohol (ETOH), substance use, and nicotine, ensuring rapid screening and proper identification of individuals at risk for substance related harm ensuring alignment with trauma clinical guidelines
• Participates in the Clinical Ethics Committee as assigned
• Familiar with standard concepts, practices, and procedures within the field
• Relies on experience and judgment to plan and accomplish goals
• Regular and predictable attendance is an essential job function
• Competent to meet age specific needs of the unit assigned
Requirements and Minimum Qualifications
• Master of Social Work (MSW) from a university accredited by the Council on Social Work Education required, Licensed Master Social Worker (LMSW) required
• 5 years’ professional medical social work experience, including work in an acute care setting, with working knowledge of mental health, chemical dependency and transitional care including insurance regulations
• 3 years’ leadership experience, including qualification as an “approved supervisor” in the State of Idaho
• Knowledge and demonstrated competencies with DSM-5 as it applies to mental health assessments in addition to crisis management, conflict resolution and stress management
• Experience facilitating transitions of care and crisis intervention
• Preferred experience in the design and implementation of a palliative care initiative spanning acute care and ambulatory settings and knowledge of end-of-life planning and Advance directives
