The Data Clerk will gather this medication information from:
-
Interview of the patient and/or significant other
-
The patient’s home medication list
-
Home medications in the possession of the patient
-
Long term care medication administration record
-
Physician clinic list
-
Retail pharmacies, as available.
-

The Data Clerk will document the list of current medications and work with the pharmacist to complete an initial review.
-
High School Diploma or equivalent.
-
Completion of Pharmacy Technician Training Course.
-
Computer skills are required.
-
Excellent written and verbal communication skills.
-
Pharmacology education or pharmacy experience preferred.
-
Excellent typing skills preferred.
-
Illinois registration as a Pharmacy Technician
-
Certification by the National Pharmacy Technician Certification Board or equivalent unless exempted by the state of Illinois due to Grandfathering
