Schedule: Full-Time | Mon – Fri, 8:00 AM – 5:00 PM
Hiring Range: $157,089.94 - $161,017.19 per year

What You’ll Do:
As a Clinical Pharmacist and key member of our primary care team, you will play a vital role in optimizing patient outcomes through direct care and provider collaboration:
- Collaborate with providers to deliver medication reconciliation, pain management, and chronic disease support
- Counsel patients on safe, effective medication use -- both in clinic and at community outreach locations
- Monitor therapeutic progress and document care in the electronic health records (EHR) system
- Provide education for medical residents, providers, and the clinical team
- Support programs like diabetes care, tobacco cessation, and anticoagulation therapy
- Research and recommend evidence-based formulary changes to enhance access and care
What You Bring:
- Doctor of Pharmacy (PharmD) and current WA State Pharmacist license and must meet one of the below requirements:
- Completion of an ambulatory practice pharmacy residency program
- -OR- (2) years of clinical ambulatory experience
- -OR- (10) years of pharmacy practice experience
