About This Position

  • Position Summary
    • Directs and oversees the effective management of strategic projects and initiatives related to clinical integration, value based care, and population health for Med Center Health and Med Center Health Partners. Responsible for leadership, oversight, operational support, and organizational alignment of clinical integration and population health initiatives. Provides leadership, guidance, and work prioritization to employees involved in day-to-day work or special projects related to areas of responsibility. Coordinates optimum allocation of financial and other organizational resources to project teams and provides oversight and supervision to project team leaders. Establishes or approves project timelines and milestones, tracks progress, monitors and communicates project status regularly, and identifies and resolves issues related to projects. Partners with physicians and other stakeholders to locate opportunities to develop and prioritize programs/projects according to relevant measurement criteria. Ensures that all projects are delivered within the defined scope, quality, time and cost requirements. Participates in the final selection of vendor partners.
  • Minimum Qualifications
    • Work Experience
      • 4 years of experience directly related to supervision of value based care, population health, strategically complex projects in the areas of quality, and operations improvement including but not limited to process redesign, care transformation, performance measurement/management, quality improvement and capacity management required.

    • Education
      • Bachelor's degree in Business Administration, Healthcare Administration, or related field required.

      • Responsible for leadership, oversight, operational support, and organizational alignment of clinical integration and population health strategic initiatives across Med Center Health and Med Center Health Partners.
      • Participates in the development of strategic business plans for clinical integration and population health activities.
      • Takes a lead role in organizing clinical integration and population health efforts to advance the mission and vision of the organization by directing and implementing the strategic plan including but not limited to payor value-based contracting, development and maintenance of the network's Quality/Clinical Integration Program, and the network's Distribution Plan.
      • Supports the leadership team to facilitate development and ongoing maintenance of clinical integration and population health strategic roadmap.
      • Supports activities to advance the organization's strategic development, including the ongoing implementation of core initiatives and new models.
      • Systematic and standardized monitoring and evaluation of clinical integration and population health operational and performance outcomes.
      • Works in a cross-functional environment with leaders and staff across all Med Center Health entities to improve effectiveness, efficiency and overall performance of clinical integration and population health initiatives.
      • Identify and help develop solutions for organizational/resource barriers that prevent implementation of initiatives.
      • Establishes and facilitates monitoring, distribution and communication of key operational metrics associated with clinical integration and population health strategic roadmap.
      • Ensures that changes to business functions, processes, financials and organization structure are optimized with the appropriate workflows, technology and people to support clinical integration and population health strategies.
      • Prepares and delivers regular, effective communications to ensure leadership across Med Center Health entities are adequately apprised of clinical integration and population health initiative statuses, successes, risks and issues.
      • Collaborates with senior leaders across Med Center Health to leverage tools and technologies to resolve issues and improve performance, process and capabilities as well as manage the culture shift and transition across the community, from traditional fee-for-service to value-based, coordinated patient care.
      • Oversees Med Center Health Primary Care's value based agreements and ensures targets are met and issues impacting ability to meets targets are resolved.
      • Participates on community-based coalitions as necessary to identify resources to support activities; and promotes collaborative tactics among and between community partners and clinical services.
      • Takes a lead role in understanding and helping to address social determinants of health that are barriers to health and well-being.
