EPIC Health is seeking a bilingual Advanced Practice Provider (APP) to join our Chicago team on a locum, contract, or contingent basis. We are open to different employment arrangements depending on the provider's availability and our clinic's needs.

We are looking for a Family Nurse Practitioner or Physician Assistant who is passionate about patient care, comfortable working in a fast-paced outpatient environment, and able to provide high-quality care to both English- and Polish-speaking patients.

• Provide comprehensive patient care: Examine patients, obtain health histories, perform physical examinations, and assess patient conditions.
• Diagnose and treat: Evaluate and manage acute and chronic medical conditions within your scope of practice.
• Develop treatment plans: Establish individualized treatment plans and prescribe medications and therapies in accordance with applicable Illinois regulations and your professional scope of practice.
• Order and interpret diagnostics: Order and review laboratory tests, EKGs, imaging, and other diagnostic studies as appropriate.
• Provide preventive care: Counsel patients on health promotion, disease prevention, growth and development, family planning, and other preventive healthcare needs.
• Perform clinical procedures: Provide procedures such as injections, immunizations, wound care, suturing, and other services within your scope of practice.
• Coordinate patient care: Manage referrals, follow-up care, diagnostic results, and ongoing treatment plans.
• Communicate with patients and families: Provide clear, compassionate care to both English- and Polish-speaking patients.
• Collaborate with the clinical team: Work closely with physicians, medical assistants, technicians, and other healthcare professionals to ensure coordinated patient care.
• Maintain documentation: Complete accurate and timely clinical documentation in accordance with organizational and regulatory requirements.

• Active Illinois PA or APRN/FNP license required.
• Board certification appropriate to your professional designation required.
• Master's degree from an accredited PA or advanced nursing program required.
• Bilingual in English and Polish required.
• Primary care experience preferred; pediatric experience is a plus.
• Strong clinical assessment, diagnostic, and patient-management skills.
• Excellent communication and interpersonal skills.
• Ability to work independently while collaborating effectively with physicians and the broader clinical team.
• Comfortable working in a fast-paced outpatient environment.
• Commitment to compassionate, patient-centered, and culturally responsive care.

• Locum assignments for temporary or short-term coverage.
• Contract arrangements for providers seeking a defined engagement.
• Contingent/per-diem opportunities for providers who prefer flexible scheduling and occasional shifts.

The specific arrangement, schedule, and compensation will be determined based on the provider's qualifications, availability, and EPIC Health's staffing needs.

For providers interested in a long-term opportunity, there may also be the potential to transition into a full-time position if the provider and EPIC Health mutually determine that a permanent arrangement is a good fit.
