High Desert PACE is looking for a dynamic, field-based Business Development Representative to champion our mission of delivering integrated care to seniors. If you are a self-directed healthcare professional with a background in post-acute care, senior services, or managed care, this role offers the autonomy to make a tangible impact.

In this position, you will own territory strategy cultivating vital community referral relationships, educating partners on our unique model of care, and identifying eligible seniors who would thrive in our program. By building a consistent referral pipeline and meeting monthly enrollment targets, you’ll directly connect seniors with the comprehensive care they deserve. This is a salaried role featuring a monthly incentive structure tied directly to your enrollment success.

Prior PACE (Program of All-Inclusive Care for the Elderly) experience, hospice, or senior care is preferred.

Requirements:

Key Responsibilities

  • Territory Strategy: Develops and executes a comprehensive outreach strategy for an assigned territory to meet monthly referral and enrollment expectations.
  • Relationship Management: Expands and sustains a network of active referral sources through strategic account management and consistent follow-up.
  • Community Partnership: Establishes and maintains strong working relationships with various healthcare and community partners, including:
  • Hospitals and outpatient clinics
  • Skilled nursing facilities and post-acute providers
  • Independent and assisted living communities
  • Physicians and healthcare professionals
  • Faith-based and social service organizations
  • Clinical Education: Educates referral sources, prospective participants, and caregivers on the High Desert PACE model of care and specific eligibility requirements.
  • Patient Identification: Targets and identifies eligible populations (Medicaid, Medicare, Dual Eligible, Veterans, and Private Pay) who are a strong fit for the PACE program.
  • Lead Qualification: Researches new opportunities and follows up on complex leads from the call center and internal partners to determine program suitability.
  • Activity Documentation: Maintains accurate and timely documentation of all outreach and enrollment activities within Monday.com.
  • Performance Standards: Meets or exceeds assigned productivity, activity, and enrollment performance expectations.

Required Qualifications

  • Experience: Minimum of 3 years of proven success in healthcare-focused outreach or marketing, including:
  • B2B: Hospitals, providers, post-acute settings, and senior living.
  • B2C: Direct interaction with individual patients and family decision-makers.
  • Insurance Knowledge: Experience working with Medicare Advantage, Managed Care Medicaid, or Veterans Administration health benefits.
  • Strategic Planning: Demonstrated ability to build and execute outreach plans with measurable results.
  • Communication: Exceptional presentation and relationship-building skills; ability to explain complex care models clearly.
  • Tech Savvy: Proficiency using CRM systems to manage referral pipelines and document field activities.
  • Education: Bachelor’s degree in business, human services, or a related field (or equivalent relevant healthcare experience).
  • Logistics: Valid state-issued driver’s license, reliable personal transportation, and an acceptable driving record.

Preferred Qualifications

  • Bilingual: Proficiency in English/Spanish.
  • Local Network: Existing referral relationships within the High Desert region’s senior care or healthcare settings.
  • PACE Expertise: Prior experience with or strong familiarity with the PACE model of care.
  • Community Alignment: Proven experience aligning healthcare services with community needs through strategic partnerships.
