Job Description

We're looking for a compassionate, dependable, and clinically skilled Registered Nurse (RN) who thrives in a fast-paced environment and is confident responding to changing patient needs.
This position is ideal for an RN who enjoys variety, problem-solving, teamwork, and time-sensitive clinical situations while maintaining accuracy, professionalism, and patient safety.
What You'll Do
- Assess clients with substance use and mental health conditions
- Provide nursing care using the nursing process: assessment, planning, implementation, and evaluation
- Administer and document medications according to provider orders, facility policies, and Texas regulations
- Monitor and document vital signs, CIWAs, COWS, safety assessments, nutritional assessments, pain, anxiety, cravings, and other required assessments
- Respond appropriately to medical emergencies and changes in a client's physical or mental condition
- Complete medication reconciliation and maintain accurate medication records
- Perform phlebotomy, venipuncture, fingersticks, EKGs, and urine drug screens as required
- Collaborate with physicians, psychiatric providers, nursing staff, Behavioral Health Technicians, and the treatment team
- Participate in treatment team and nursing meetings
- Provide patient and family education regarding medications, wellness, and recovery
- Maintain infection control standards and required medical supplies
- Accurately document nursing care, incidents, medication errors, and patient responses
- Maintain patient confidentiality and comply with all applicable laws, regulations, and Clearfork Academy policies
- Complete required continuing education and organizational training
- Perform other duties as assigned

- Current and active Texas RN license
- At least 1 year of staff nursing experience, preferably in behavioral health or chemical dependency
- Current BLS certification
- Knowledge of medications, nursing standards, and the Texas Nurse Practice Act
- Basic understanding of substance use disorders and recovery
- Working knowledge of ASAM criteria
- Ability to perform vital signs, EKGs, breathalyzer testing, urine drug screens, venipuncture, and fingersticks
- Strong clinical judgment, communication, and documentation skills
- Ability to work independently in a fast-paced environment

The RN works in an outpatient behavioral healthcare environment serving adolescents and young adults with substance use disorders, with or without co-occurring psychiatric conditions.
This position requires the ability to work in a fast-paced environment, respond to changing patient needs, and maintain professionalism and accuracy under stress. The position may require extended periods of sitting or standing and occasional lifting or moving of up to 25 pounds.
