Perinatal Nurse Navigator

Work hours: Part Time, 24 Hours, Days
Departmental highlights:

Here's What You Need
• Bachelor's degree in Nursing required
• Master's degree in Nursing preferred
• Three years of Level III NICU experience preferred
• NRP - Neonatal Resuscitation Program upon hire required
• BCLS - Basic Cardiac Life Support upon hire required
What You Will Do
• Supports high risk mothers and families identified for navigation during pregnancy and transition to delivery or NICU admission
• Facilitates communication among maternal-fetal medicine, obstetrics, neonatology, pediatric subspecialists, palliative care, outpatient providers, labor and delivery, and NICU teams
• Ensures seamless transitions across inpatient, outpatient, and community based care settings
• Collaborates with obstetric, neonatal, surgical, nursing, social work, case management, chaplaincy, and therapy teams
• Interface with external organizations for documents needed for the coordination of activities, programs and partnerships
• Prepares and leads bi monthly Perinatal Rounds to promote communication, coordination, and proactive care planning for high risk antenatal and fetal patients
