About This Position

  • Position Summary
    • Under the general direction of the Center for Medication Management Clinic Manager, provide advanced education and care to patients receiving anticoagulant therapy, hyperlipidemia management, and diabetic education. Responsible for working with physicians within the predefined guidelines approved by the P&T Committee for management of patients referred for anticoagulation therapy, hyperlipidemia management, and diabetic education. This includes but is not limited to the following medications warfarin (Coumadin), low molecular weight heparin (LMWH), aspirin, simvastatin (Zocor), lovastatin (Altoprev, Mevacor), rosuvastatin (Crestor), fluvastatin (Lescol), atorvastatin (Lipitor), pitavastatin (Livalo), and pravastatin (Pravachol). Other medications include bile acid resins: cholestyramine, colestipol, colesevelam, and the antilipemic agent: eztimibe (Zetia). Niacin, Fish oil, and fibric acid derivatives: Fenofibrate, gemfibrozil, and fenofibric acid are several other medications. The nurse practitioner is responsible directly for the warfarin (Coumadin) dosing, patient education, scheduling of appointments, identifying drug-drug interactions and other responsibilities related to issues that arise in the management of patients’ warfarin (Coumadin) therapy, such as bleeding and scheduled procedures. The nurse practitioner is also responsible for managing a patient’s basic or advanced lipid profile by medication management and lifestyle modification. The nurse practitioner provides education and other responsibilities involving lipid management. The nurse practitioner also provides detailed diabetic education, including but not limited to, information about drug regimen, diet, and exercise. Responsible for providing tools and educational opportunities to enhance services provided by all staff members. Serves as assistant preceptor to students and residents. Participates in supervision of technical staff.

  • Core Job Functions
    • After receiving a Physician order, provides full therapy education with verbal and written communication. Assess and maintain warfarin therapy. Manage Home Health care patients regarding PTTs/INR.
    • Functions in accordance with Medical staff rules and regulations. Complete history and physicals are present for every patient. Maintains regular and frequent communication with the medical staff involved in the care of the patients.
    • Assess pharmacotherapy and individualize therapy to specific patient needs and communicate therapeutic recommendations to the Medical Staff. Establish and clarify goals and objectives of therapy in collaboration with the physician for each problem being treated. Provide patient education and counseling and participate in discharge planning for the purpose of ensuring continuity of drug therapy.
    • Supervise staff competencies for INR and Piccolo testing devices. Ensure quality controls are maintained both internal and external. Assist with ordering an maintenance of supplies.
