Job Description

  • Position Summary
    • Looking for a dedicated LPN to join our team and provide care to a designated group of patients that have been diagnosed with osteoporosis and other disorders affecting bone health. Works closely with nurse practitioner assessing, and providing and coordinating care. Must be able to obtain and keep track of pertinent labs/scans (DEXA), insurance verification, appointments and follow-up care. Utilizes the nursing process by administering injections (Prolia), providing patient education, documenting pertinent history, medication reconciliation and reassessment. In addition, will work in the infusion clinic as needed and should be proficient with intravenous access. This candidate must also be able to travel between clinics (Bluffton and Hilton Head).

    • College Diploma in Nursing - Required
  • Experience
    • 1 - 2 Years Physician Practice or Ambulatory Care Setting - Required
  • License & Certification
    • Professional License with State of Practice - Required
    • Cardiopulmonary Resuscitation (CPR) - Required
  • Core Job Functions
    • Demonstrates knowledge and skill to provide care appropriate to any age-related needs of the patient being served. Demonstrates principles of growth and development appropriate to the patient populations served. Coordinates patient care needs with team members and other appropriate departments.
    • Collaborates with RN, Healthcare team to plan/revise patient care and interventions. Contributes to formulation of nursing care for patients in consultation with the RN and in collaboration with patient, and their families using established nursing diagnosis and nursing protocols. Prioritize nursing care needs of patients.
    • Administers medication utilizing the 5 rights and available safety equipment and processes. Utilizes clinical judgment with patient assessment and prescribed therapies to ensure promotion of goal obtainment. As a leader holds healthcare providers accountable for the standard of care.
    • Utilizes Teach Back technique to assess understanding of education. Assesses needs for discharge and the best way the patient may be provided instruction. Utilizes appropriate written communication, education packets and/or audio-video instructions. Teach back performed with patients to facilitate patient outcome improvements by increasing knowledge and skills needed to improve healthy behaviors. Elicit information using effective counseling techniques. Identify factors that might affect communication or learning.
    • Uses critical thinking skills to assess and document to improve patient health outcomes. Participates as a resource in developing tools to accurately assess and document the physical, psycho-social, growth, and developmental dimensions of the patient. Reviews unit, staff performance in collaboration with the manager.
    • Complies with regulatory guidelines for work area and professional practice standards. Utilizes available safety equipment and processes for patient care. Promotes a safe and therapeutic environment for patients.
