Citizens Memorial Hospital (CMH) is recruiting a LCSW or LPC for our Behavioral Assessment Team (BAT) at the Bolivar Family Care Center in Bolivar, MO. CMH is made up of 34 primary and specialty clinics, 6 Long-Term Care facilities and 1 residential care facility. Offering acute care, emergency services, ambulatory care, pharmacies, walk-in clinics, surgeries, addiction recovery, therapies and more, CMH has its community covered!

About the department: Supporting hospital and emergency room patients of all ages, the Social Services Department is available 24/7 to offer assistance with discharge planning, resource referrals, and consultative services. Working as part of an interdisciplinary team, Social Workers and Designees provide clear communications to patients, family members, and other care team members to ensure smooth transitions and continuum of care.
A typical day will include: Conducting safety risk assessments to make appropriate recommendations and referrals for safety planning with patients and/or caregivers.
- Reviews patient charts and consults the on-staff physicians regarding clinical concerns
- Meets face-to-face with patients to conduct diagnostic assessments to include crisis evaluation using appropriate risk assessment measures
- Provides resource referrals
- Documents appropriate information with accuracy
Job Requirements:
- Education: Masters Degree in Social Work or Masters/Doctorate in Psychology
- Experience: 1-3 years behavioral health or substance abuse counseling (may include pre-degree practicum)
- Skills: Strong written and verbal interpersonal communication
- Licensure/Certification: Current Missouri LCSW, LPC, or Licensed Psychologist. We are unable to consider PLPCs or LMSWs at this time
This position is: On-call - PRN Days, 7:30am - 7pm, and is required to present within an hour of call.
