Job Description

Primary Responsibilities

As a member of the interdisciplinary team, the CNA Hospice Aide works under the general direction of the DOCS, the Patient Care Coordinator, and the Nurse Case Manager. The CNA Hospice Aide will provide personal care services to the terminally ill patient and perform related tasks in the patient/family home or assigned facility in order to increase their level of comfort.

Essential Functions

  • Receives specific written duty instructions from the supervising case manager prior to delivery of service, as well as contacting families in advance of visit to set the approximate time of arrival.
  • Assists patients with personal hygiene, including shower, tub or bed baths, mouth, skin and hair care and toileting.
  • Assists patients with daily activities to include walking, mobility in and out of bed, and assists with physical therapy and use of mechanical lifting equipment, wheelchairs, or walkers when applicable.
  • Performs blood pressure, pulse, and respiration when required.
  • Assisting patients with administration of topical medications under the direction of the RN, that the patient would normally self-administer.
  • Records and reports changes to the nurse supervisor in the patient's physical condition, behavior or appearance, and documents services delivered and observations in accordance with hospice medical records policies and procedures.
  • Assists with meal preparation and feeding, when required.

Knowledge, Skills, and Education Requirements

  • High School graduate or GED; further healthcare education preferred.
  • At least one (1) year experience in home health, hospice, oncology, or critical care preferred.
  • Current Certified Nursing Assistant (CNA) certification required.
  • Current CPR certification.
  • Demonstrated ability to assess and respond to the needs of patients and families in varied settings, communicating effectively in a sensitive manner, providing outstanding customer service.
  • Well-versed computer literacy with in-depth knowledge and experience with electronic medical records programs.
  • Current driver's license in the state of Virginia as work requires the employee to travel outside the office and provide their own transpiration with mileage reimbursement from the agency.

Work Environment & Physical Demands

Situated in a professional medical environment and/or in patient homes of unknown environments yet may have the potential for locations in high crime areas, poor environmental conditions, and non-handicapped accessible sites. This role is frequently active with occasional lifting and/or carrying up to 50 lbs. The individual performing this job may reasonably anticipate coming into contact with human blood and other potentially infectious materials. Individuals in this position are required to exercise universal precautions. Frequent local travel with mileage stipend as required for patient care.
