Hours Per Week: 36

Schedule: Day/Evening/Night, 12 hour shifts, Start times include: 7a, 11a, 3p, 7p, 11p

SUMMARY

The Care Navigator functions as a core member of an Integrated Care team that includes the Primary Care Provider (PCP), Behavioral Health Clinician (BHC), and psychiatric provider. The Care Navigator is responsible for coordinating and supporting Mental Health care within the clinic and for coordinating referrals to clinically indicated services within and outside of the clinic. The Care Navigator may provide evidence-based treatments to patients or collaborate with other Mental Health providers when such treatment is indicated. This role will report to the Manager, Behavioral Health/Primary Care Integration or designee.

RESPONSIBILITIES

  • Support and coordinate behavioral health care with the patient, patient’s Primary Care Provider (PCP), BHC, psychiatric provider and other Integrated Care team members.
  • Screen and assess patients for common mental health and substance abuse disorders. This may involve completing or assisting with completion of identified screening tools utilized in practice.
  • Provide patient education about common mental health and substance abuse disorders and available treatment options.
  • Monitor patients (in person or by telephone) for changes in clinical symptoms and treatment side effects or complications.
  • Support psychotropic medication management prescribed by PCPs and/or psychiatric provider, focusing on treatment adherence, side effects and other complications, and effectiveness of treatment.
  • Provide brief interventions using evidence-based techniques such as Behavioral Activation, Problem-Solving Treatment, Motivational Interviewing, or other treatments appropriate for primary care settings.
  • Provide or facilitate referrals to mental health clinic/substance use disorder treatment programs as clinically indicated.
  • Participate in regularly scheduled Integrated Care team meetings and communicate treatment recommendations to team members and outside collaborators as indicated.
  • Facilitate patient engagement and follow-up in care.
  • Maintain a registry of patients in the Integrated Care track that includes engagement, measurement- based care outcomes (PHQ-9) results and other items being used by the team to track and manage care.
  • Facilitate the Integrated Care team’s review of care of patients not responding optimally to treatment that may lead to management changes in medications or psychosocial treatments, or appropriate referrals for additional services.
  • Handle confidential information with discernment and sensitivity.

REQUIRED QUALIFICATIONS

  • Minimum of 2 years of experience in Human Services related field, medical related field, customer service, client/patient interfacing experience required; OR
  • Associate’s degree required

PREFERRED QUALIFICATIONS

  • Bachelor's degree
  • Psych experience

EDUCATION:

  • AS (Required)

PHYSICAL REQUIREMENTS: S - Sedentary Work - Exerting up to 10 pounds of force occasionally Sedentary work involves sitting most of the time, but may involve walking or standing for brief periods of time. Jobs are sedentary if walking and standing are required only occasionally and all other sedentary criteria are met.

PAY RANGE: $20.00 - $25.00

The listed base pay range is a good faith representation of current potential base pay for successful applicants. It may be modified in the future. Pay is determined by factors including experience, clinical licensure date, relevant qualifications, specialty, internal equity, location, and contracts.
