Primary Health Care (PHC) was founded in 1981 by Dr. Bery Engebretsen in Des Moines, IA. Our mission has remained unchanged since that time, to provide healthcare and supportive services to all, regardless of insurance, immigration status, or ability to pay.

Based on the needs of the communities we serve, PHC offers a spectrum of medical and dental services including family practice, behavioral health, HIV care and services, and pharmacy. PHC’s Homeless Support Services is the entry point for serving people experiencing homelessness in Polk County. Enabling services are available to help patients with benefits enrollment, case management, transportation, translation, and patient education. We currently have locations in Ames, Des Moines, & Marshalltown.

As a Prevention Services Manager, you will be responsible for providing leadership and coordination of prevention programming, including mobile health unit outreach, HIV, STI, HCV screening, PrEP services, and condom distribution. Provides supervision of program Prevention Specialists. Serves as a liaison and provides coordination with other departments of PHC, community partners, and funding and oversight agencies to integrate prevention services across PHC and within the community. Works closely with PHC’s HIV Clinical Director to coordinate clinic and outreach activities. Demonstrates iCare values in daily work.

What You Will Do

1. Manages day-to-day functioning of outreach activities with PHC’s mobile health unit including scheduling, inventory, policy/procedure changes and quality control, ensuring maintenance and upkeep of mobile unit. Manages and coordinates clinical components of the prevention program including rapid HIV and HCV testing, STI screening, pre-exposure prophylaxis (PrEP) services and condom distribution program.
2. Supervises the program’s prevention specialists to facilitate the continuum of care. Indirectly supervises other prevention and outreach related staff.
3. Interfaces with all other PHC departments, clinics, and outside state and local agencies regarding any HIV and STI prevention activities in order to integrate routine HIV and STI testing, PrEP, and other initiatives. Establishes working relationships with community partners, other PHC departments, and funding plus oversight agencies. Participates in appropriate advisory boards, task forces, coalitions, and committees.
4. Facilitates program development, leading program staff in goal setting and implementation based on the project work plan and other identified program objectives. Monitors and evaluate completion of goals, using outcomes to move the team forward. Coordinates quality management efforts for assigned programs by developing and maintaining program policies and procedures, participating in quality assurance activities, chart reviews, program and staff evaluation, staff development trainings, and outcome analysis.
5. Develops and maintain required records, reports, and program budgets including administrative tasks such as completing monthly reporting, managing program budgets, etc. Develops and submits grant applications and reports for program continuation in accordance with Federal regulations. Maintains and manages program databases and EMR and trains all staff accordingly on its use and application.
6. Supports and empowers clients throughout health care continuum to include holistic counseling, problem solving “barriers to care” issues and coaching clients as needed re: harm reduction, treatment and medication adherence, healthy life choices, and self-help behaviors. Performs prevention interventions and refers or monitors appropriately. Provides HIV education to clients/families/partners, internal staff, and external agencies in an organized and appropriate manner. Assists clients in accessing support services by serving as a liaison with other community agencies. Manages client referrals and follow-up to other specialty medical, mental health, substance abuse and sexual assault/domestic violence services.
7. Backs up Prevention Specialist team to provide HIV, STI, and Hepatitis C testing for individuals at increased risk for contracting these viruses and bacterial infections per PHC policy and procedures. Identifies underlying risk factors and creates risk-reduction goals through counseling. Provides referrals and support.
8. Assists in projects and performs other duties as assigned.

Qualifications You Need to Bring

Required:

• Bachelor’s degree in psychology, counseling, social work, mental health, human services or other related field or equivalent combination of education and experience.
• Minimum 2 years of experience in social work, public health or related field
• Minimum of 1 year experience in a leadership role
• Willingness and ability to learn computer software applications; proficient with Microsoft Outlook applications and ability to utilize program management and database systems.
• Effective verbal and written communication skills.
• Leadership and management skills with ability to guide, coach and develop others.
• Exhibits professionalism when interacting with others while maintaining composure and demonstrating empathy.
• Organization, prioritization and time management skills with ability to multi-task in a fast-paced environment.
• Team oriented with ability to work collaboratively and build/maintain professional relationships at all levels.
• Strong detail orientation with high degree of accuracy.
• Reliable transportation required.

Licenses & Certifications: Approved phlebotomy certification; current Mandatory Reporter Training- Child and Dependent Adult Abuse recognized by the Iowa Department of Public Health; successful completion of a training program in the driving and maintenance of a mobile health unit.

Preferred:

• Master’s Degree in Social Work or related field
• Previous experience in public and grant funded programs.
• Previous experience in a community health center or other medical clinic setting.
• Previous experience in HIV prevention and care, case management, counseling and/or implementing health behavior interventions, or other community health programs.
• Previous experience working with prioritized populations including gay, bi, and other men who have sex with men, transgender individuals, and disproportionately affected communities of color.
• Previous experience coordinating group efforts or similar activities.
• Bilingual, verbal and written language proficiency in English and Spanish.
• Clinical skills: finger prick, swab collection, urine processing, phlebotomy.
• Working knowledge of operations and maintenance of a commercial vehicle.

Your experience and skills determine your base pay. The hiring range for this position is typically $47,120 - $58,900 annually. Candidates with extensive work experience related to this position may be considered for additional compensation up to the pay grade maximum of $70,680 annually.
