Job Title: RN or LPN - Community Nursing

Reports to: RN Program Manager – Integrated Healthcare

Location: Heritage Behavioral Health Center, Decatur, IL (includes community-based work in surrounding areas)

Schedule: Full-time, Monday–Friday (8:00 AM – 5:00 PM)

Position Overview:

Heritage Behavioral Health Center is seeking a Registered Nurse (RN) or Licensed Practical Nurse (LPN) to support care delivery across our integrated care system, including behavioral health, primary care, and community-based services.

This role provides direct nursing support in both clinic and community settings, working closely with providers and interdisciplinary teams to support individuals with serious mental illness and chronic health conditions. This nurse plays a key role in improving access to care, supporting medication management, and promoting overall health and wellness for the individuals we serve.

Core Responsibilities:

Clinical & Community-Based Care

  • Provide direct nursing support to a psychiatric provider in a clinic setting, including patient assessments, documentation, medication administration, and coordination of care
  • Deliver nursing services in community settings, including group homes and client residences
  • Conduct well-being checks, basic nursing assessments, and health monitoring in community environments
  • Accurately obtain and document vital signs, physical observations, and health concerns

Medication Management & Support

  • Administer prescribed medications (including long-acting injectables) as ordered
  • Monitor for adverse reactions and report concerns toappropriate medicalstaff
  • Provide medication education to support safe and effective medication use
  • Coordinate with pharmacy to ensuretimelyrefills and medication availability

Care Coordination & Communication

  • Communicate clearly with providersregardingchanges in patient condition or care needs
  • Facilitate referrals and linkage toappropriate medicaland behavioral health services
  • Collaborate with team members to address urgent medical needs and supporttimelycare
  • Assistwith triage support and coordination of follow-up care

Documentation & Compliance

  • Ensuretimely,accurate, and complete documentation in the electronic health record
  • Maintain organization of supplies, medications, and nursing tools across settings
  • Support adherence to organizational policies, procedures, and care standards

Education and Experience:

  • Active, unrestricted Illinois Registered Nurse (RN) or Licensed Practical Nurse (LPN) license
  • Experience in behavioral health, primary care, or community-based care preferred
  • Ability to work independently and as part of a multidisciplinary team
  • Strong organizational skills and attention to detail
  • Compassionate and empathetic approach to patient care

Compensation & Benefits

Salary Range: $26-$33 per hour (based on experience)

Sign-On Bonus: RNs are eligible for a $5,000 sign-on bonus and LPNs are eligible for a $2,500 sign-on bonus, each with a 1-year service commitment. Bonuses are paid out in increments within the first 6 months of employment.
