Job Title: Weeknight On-Call Admissions RN
Job Type: Evenings/overnight, Monday-Thursday

Job Summary:
As an On-Call Admission nurse, you'll conduct comprehensive patient assessments, facilitate seamless admission procedures, and deliver compassionate care coordination, ensuring the best support for hospice patients and their families.
Responsibilities:
- Coordinates and oversees all aspects of new patient admissions
- Collaborates with patient care teams to ensure seamless communication
- Educates families on patient condition, needs, and care
- Performs head-to-toe physical assessments
- Timely and accurately completes documentation
- Back-up On-Call nurses for PRN visits
- Other duties as assigned
Licensure:
Current State RN license.
Current driver’s license.
Current CPR Certification.
Experience:
Two (2) years' experience as a Nurse in a clinical care setting and one (1) year full-time hospice preferred.
Skills:
Nursing skills as defined as generally accepted standards of practice.
Knowledge of state and federal regulations.
Ability to work independently and make sound decisions.
Strong attention to detail and organizational habits.
Excellent verbal and written communication skills.
Good interpersonal skills.
Transportation:
Reliable transportation and valid auto liability insurance.
