The Case Manager SUDP - Community Mental Health provides direct support and case management services for individuals with mental health and substance use challenges. This role focuses on recovery-oriented care, community integration, and helping clients develop the skills and resources needed to live as independently as possible.

• Serve as a primary support and case manager for individuals experiencing mental health conditions and substance use challenges.
• Provide community-based services focused on recovery, rehabilitation, harm reduction, and long-term stability.
• Assist clients with developing and achieving individualized recovery goals through person-centered care planning.
• Coordinate services and connect clients with appropriate community resources, healthcare providers, and support networks.
• Support clients in improving daily living skills, independence, social connections, and overall wellness.
• Collaborate with multidisciplinary teams to provide comprehensive behavioral health support.
• Maintain accurate documentation, case notes, and service records according to program standards.
• Provide ongoing guidance, encouragement, and advocacy to individuals navigating recovery journeys.
• Transport clients when required using a personal vehicle that meets safety and insurance requirements.
• Participate in scheduled shifts, including occasional evenings, weekends, or holidays based on program needs.

• High school diploma or GED required.
• Active Washington State Substance Use Disorder Professional (SUDP) credential required.
• Minimum of six months of experience working in an addictions treatment or substance use setting.
• Experience providing case management, behavioral health support, or community-based services preferred.
• Strong interpersonal skills with the ability to build trusting relationships with clients.
• Ability to work collaboratively within a multidisciplinary care team.
• Commitment to culturally responsive, inclusive, and person-centered services.
• Ability and willingness to use a personal vehicle for work-related transportation.
• Valid personal vehicle insurance with the candidate listed as a driver.
