ESSENTIAL DUTIES & RESPONSIBILITIES:

- Provide staff development and training to both new and existing employees.
- Instruct and certify new employees in receiving their Level 1 Medication Aide certificate, teaching Personal Care, and QuickMAR.
- Provide additional remedial instruction to staff who may need additional support with skills or concepts previously taught.
- Conduct additional training sessions or in-services, both at the main office and in the community.
- Perform Quality Assurance tasks to enhance the quality of services for the people we support.
- Track, monitor, evaluate, and report the data collected from chart or documentation audits.
- Along with the Director of Nursing (D.O.N.) and nursing department apply audit data to improve clinical practices and processes to enhance quality outcomes.
- Use assessment tools to follow a well-coordinated healthcare plan to coincide with the person we support’s person-centered plan.
- Complete ongoing nursing assessment of people we support as needed.
- Participate in person-centered plan when appropriate.
- Obtain physician orders and ensure their execution as requested.
- Review medications as needed to ensure that all medications match POs and are not expired prior to administration.
- Update RN and/or Director of Nursing with any changes in health status of any resident.
- Communicate effectively and work cohesively with waiver supervisory staff to ensure appropriate and prompt medical care.
- Knowledgeable about the various aspects of the health care delivery system required to ensure compliance with all federal, state, and local government laws and guidelines as well as Sunnyhill policies and procedures.
- Ensure medical equipment and supplies are available for use in the homes and assist in providing in-service training for all staff in the homes on equipment and delegated training.
- Provide consultation on discharge or admission to the hospital or other facility.
- Serve as liaison between hospital and staff regarding progress of people we support.
EDUCATION & EXPERIENCE
- Licensed Practical Nursing (LPN) or Licensed Vocational Nursing(LVN) Diploma
- High School Diploma or GED
- Minimum of 3 years of LPN experience or similar experience, preferred.
- Behavioral health experience, preferred.
- Patient physical assessment experience.
REQUIRED SKILLS/ABILITIES & QUALIFICATIONS
- Experience administering Injections, G-Tube and foley catheter care, and other clinical skills preferred.
- Patient and/or staff education.
- Proficient computer skills required.
- Electronic Medication Administration.
- Experience in Quality Assurance, preferred.
- Excellent verbal and written communication.
- Ability to prioritize tasks and function well in a high-paced (at times stressful) environment.
- Valid driver’s license and auto insurance (use of personal vehicle required at times).
- Acceptable background checks.
PHYSICAL REQUIREMENTS
- Able to stand, walk, and sit four (4) hours at a time.
- Repetitive bending and reaching up to two (2) hours a day.
- Able to lift 25 pounds from floor to waist.
