Position Summary

The Licensed Vocational Nurse (LVN) provides skilled nursing care under the direction of the Registered Nurse (RN) and in accordance with the physician's Plan of Care. The LVN delivers compassionate, patient-centered care while promoting safety, independence, and improved health outcomes.

Assignments may include care provided in:

  • Private Homes
  • Hospitals
  • Skilled Nursing Facilities
  • Assisted Living Communities
  • Other Healthcare Settings as Assigned
Essential Responsibilities
  • Provide one-on-one skilled nursing care in accordance with the physician's Plan of Care.
  • Deliver specialized nursing care to medically complex and respiratory-compromised patients, including the management of tracheostomy and ventilator-dependent patients, while ensuring patient safety and adherence to physician orders.
  • Perform comprehensive patient assessments and monitor changes in condition, promptly reporting significant findings to the supervising RN and physician.
  • Administer prescribed medications, treatments, and therapies within the LVN scope of practice.
  • Monitor and document vital signs, oxygen saturation, respiratory status, and overall patient progress.
  • Provide tracheostomy care, suctioning, airway management, and ventilator monitoring based on established clinical competencies and physician orders.
  • Manage enteral feeding (G-tube/NG tube), urinary catheter care, wound care, and other skilled nursing procedures as ordered.
  • Recognize and respond appropriately to respiratory distress, changes in patient condition, and emergency situations.
  • Educate and reinforce patient and family instruction regarding disease management, respiratory equipment, medications, infection prevention, and safety in collaboration with the supervising RN.
  • Maintain accurate, timely, and complete clinical documentation in accordance with agency, state, and federal guidelines.
  • Follow all infection prevention, OSHA, and patient safety standards, including proper use of Personal Protective Equipment (PPE).
  • Collaborate with physicians, Registered Nurses, respiratory therapists, therapists, caregivers, and other members of the interdisciplinary care team to ensure optimal patient outcomes.
  • Maintain patient privacy and confidentiality in compliance with HIPAA regulations.
  • Participate in required orientation, annual competencies, continuing education, and clinical training.

Requirements: Minimum Qualifications
  • Current Texas Licensed Vocational Nurse (LVN) license in good standing.
  • Current CPR/BLS Certification.
  • Minimum one (1) year of nursing experience preferred.
  • Previous Private Duty Nursing, Home Health, Pediatric, Long-Term Care, Rehabilitation, or Hospital experience preferred.
  • Experience with tracheostomy, ventilator, feeding tube, wound care, or high-acuity patients is highly desirable (training provided based on patient assignment).
  • Valid Texas Driver's License.
  • Current automobile insurance.
  • Reliable transportation.
  • Ability to travel throughout the Sherman and Denison service area.

Physical Requirements

The position requires the ability to:

  • Stand, walk, bend, kneel, and lift up to 50 pounds with or without reasonable accommodation.
  • Safely transfer and reposition patients.
  • Respond quickly to patient needs and emergencies.
  • Provide extended periods of direct patient care.
