Boynton Beach, Florida 33437We are seeking a compassionate and detail-oriented Care Coordinator to join our growing healthcare team. This role is essential in ensuring patients receive high-quality, coordinated care while feeling informed, supported, and confident throughout their healthcare journey.The ideal candidate is a strong communicator with a patient-first mindset who thrives in a fast-paced medical environment and is passionate about delivering exceptional service and care coordination.Key Responsibilities
- Develop and coordinate patient care programs to support quality outcomes and continuity of care
- Assist in managing patient care plans and ensuring patients understand their treatment options, medications, and care instructions
- Support the care team in establishing quality assurance goals and implementing best practices
- Assess and screen patients while providing education regarding medical procedures and healthcare processes
- Coordinate referrals for continuing care and connect patients with appropriate community resources
- Collaborate with patients, families, providers, and clinical staff to help establish individualized care goals and treatment plans
- Assist patients with identifying resources or funding options for special procedures or additional care needs
- Maintain accurate documentation and ensure compliance with privacy and confidentiality standards
- Demonstrate professionalism, compassion, critical thinking, and teamwork in all patient and staff interactions
- Perform additional duties as assigned
- Minimum of 3–5 years of experience in a medical or healthcare setting required
- At least 2 years of experience as a Medical Assistant required
- Knowledge of Athena EMR, CPT coding, and ICD-10 coding preferred
- Strong interpersonal, verbal, and written communication skills
- Ability to work collaboratively within a team-oriented environment
- Understanding of healthcare privacy standards and data confidentiality practices
- Strong organizational skills with the ability to manage multiple priorities simultaneously
- Flexible and adaptable in handling changing daily demands and patient needs
