Job Summary:

The Physical Therapist Assistant (PTA) provides physical therapy interventions to PACE participants at Jordan Valley Senior Care under the direction and supervision of a licensed Physical Therapist (PT), consistent with the participant's individualized plan of care developed and approved by the Interdisciplinary Team (IDT). The PTA helps participants maintain or improve mobility, strength, balance, and functional independence so they can continue living safely in the community, which is the central goal of the PACE model. Services are delivered at the PACE center, in participants' homes, and in contracted facility settings as directed.

Key Responsibilities:

  • Implements physical therapy treatment interventions established in the plan of care under the direction of the supervising PT, including therapeutic exercise, gait and balance training, transfer training, and functional mobility activities.
  • Administers modalities and physical agents (e.g., therapeutic heat/cold, electrical stimulation) only as specifically delegated by the supervising PT and within the scope permitted by Missouri physical therapy practice law.
  • Monitors and records participant response to treatment, including vital signs, tolerance, pain levels, and functional changes, and reports significant changes to the supervising PT promptly.
  • Instructs participants, family members, and caregivers in home exercise programs, use of assistive and adaptive equipment, and safe mobility techniques to support function between visits.
  • Assists in measuring and fitting participants for ambulation aids, orthotics, wheelchairs, and other durable medical equipment as directed by the supervising PT.
  • Recommends adjustments to treatment activities to the supervising PT based on observed participant response; does not independently alter the plan of care.
  • Maintains a safe treatment environment, including proper use and maintenance of therapy equipment and adherence to infection control and fall-prevention practices.
  • Participates in discharge planning and transition of care activities, including communication with home care, transportation, and contracted facility staff.
  • Completes timely, accurate documentation of each treatment session, including objective measures of progress toward IDT-established goals, for clinical and billing purposes.
  • May assist with the ordering and management of DME delivered to participants at the Center and in the home, as directed by the PT.
  • Other job duties as assigned.

Requirements:

Required Qualifications:

  • Minimum of an Associate's degree from a physical therapist assistant program accredited by the Commission on Accreditation in Physical Therapy Education (CAPTE), or equivalent as recognized by the Missouri Board of Healing Arts.
  • Current, active license or certification as a Physical Therapist Assistant in the State of Missouri, in good standing, maintained throughout employment.
  • Current BLS certification required within 90 days of hire.
  • Have one year of experience working with a frail or elderly population or, if the individual has less than one year of experience but meets all other requirements must receive appropriate training from JVSC on working with a frail or elderly population upon hiring.

Preferred Qualifications:

  • Prior experience working with older adults, individuals with chronic illness or disability, or in a home- and community-based, geriatric, or long-term care setting is preferred.
  • PACE or interdisciplinary team-based care experience is a plus but not required.
