Job Summary:
The Family Nurse Practitioner (FNP) is a critical medical provider during our second shift operations and serves as a clinical anchor between the daytime and overnight medical teams. This role involves conducting medical evaluations, managing withdrawal symptoms, coordinating with nursing for ongoing medical care, and supporting integrated treatment plans in collaboration with our PMHNP and Medical Director.
This role is ideal for a clinician with strong primary care and substance use disorder experience who excels in a structured, fast-paced behavioral health environment. The FNP plays a key role in ensuring continuity of medical care across shift transitions and supports safe, effective medical oversight.
Essential Duties and Responsibilities:
Medical Care & Patient ManagementPerform comprehensive admission history & physicals (H&Ps) for new patients in detox and residential levels of care.
Conduct routine sick calls, follow-up assessments, and monitor vital signs, labs, and medication responses.
Manage withdrawal symptoms using established detoxification protocols in collaboration with the PMHNP and nursing team.
Provide urgent but non-emergency medical care for conditions arising during the evening shift.
Coordinate with primary care or specialty providers as needed for patients with complex medical conditions.
Support discharge planning, including medication reconciliation, prior authorizations, and aftercare instructions.
Ensure that patient medication regimens are cost-effective, accessible post-discharge, and clinically appropriate.
Clinical Documentation & Compliance
Complete timely and thorough documentation in the Kipu EMR, including H&Ps, medication orders, and progress notes.
Ensure documentation is aligned with ASAM, TJC, and Maryland Department of Health standards.
Participate in performance improvement (PI) initiatives and assist in quality assurance or peer review when requested.
Team Collaboration
Collaborate closely with the PMHNP, Director of Nursing, case managers, and clinical team to align medical care with psychiatric and therapeutic goals.
Communicate relevant changes in medical status or risk factors to appropriate departments.
Participate in evening handoff reports and maintain communication logs to support overnight continuity of care.
Patient & Family Education
Educate patients and families on medical diagnoses, treatment plans, medication use, and harm-reduction strategies.
Model trauma-informed and recovery-oriented communication practices with patients.
Other duties as assigned
Required Knowledge, Skills, and Experience:
Current, unrestricted FNP license in the State of Maryland
National Board Certification as a Family Nurse Practitioner (FNP-BC or equivalent)
Active DEA License (or willing to obtain)
BLS Certification (required); ACLS preferred
Minimum 1 year of experience as an FNP in primary care, urgent care, internal medicine, or substance use treatment setting
Familiarity with medically supervised withdrawal management and co-occurring medical/psychiatric conditions strongly preferred
Experience using Kipu EMR or other substance use documentation systems is a plus
Strong diagnostic and clinical reasoning skills
Proficient in interpreting labs, EKGs, and basic radiological reports
Ability to manage complex patient presentations with limited onsite ancillary services
Excellent communication, collaboration, and time-management skills
Comfort working in a fast-paced, mission-driven inpatient setting
Compensation:
$70-$100 per hour, depending on experience
