Position: Wound Care Provider Lead - Nurse Practitioner or Physician Assistant

The Provider Lead will be responsible for managing a team and territory, identifying the need for growth hires, coaching team members, rebalancing patient panels, as well as various other management responsibilities. The position requires providers to spend 60% of their time seeing patients while managing a reduced patient panel. This approach is intended to support providers in maintaining their clinical skills and expertise while also taking on leadership responsibilities.
As a Wound Care Provider Lead, you will play a key role in supporting the professional development of our wound care providers and promoting a culture of excellence and continuous improvement. You will be responsible for working closely with our providers to identify areas for growth and development, providing coaching and mentoring, and helping to build a supportive and collaborative team environment. This role presents a unique opportunity to support the specialized care required for wound care management across our population.
Note: this position is not intended to take on the responsibility of coaching or managing providers on all elements of success within the Bloom model. Where appropriate, coordinate and pull-in content experts on subjects including clinical matters, population health, HR, etc.
Key Responsibilities:
The salary for this position is competitive and commensurate with experience. The total compensation pay range for this role in the state of Colorado typically falls between $118,000 - $140,000 annually with the potential for performance-based bonuses and other benefits. Actual compensation may vary based on factors such as qualifications, experience, and location within the state.

Key Responsibilities:
- Oversee and manage a team of providers, including interviewing and evaluating new provider hires.
- Manage a territory, identifying the need for growth hires, and rebalancing patient panels.
- Manage team engagement by providing regular feedback, coaching, and recognition.
- Communicate company updates and initiatives to the team and ensure compliance with Bloom best practices.
- Support the coaching and onboarding of new providers to the team.
- Mobilize resources for providers including clinical, population health, HR, and ambassador support.
- Serve as a liaison between providers, clinical leadership, support, ambassadors, hospice, and leadership.
- Identify opportunities to improve clinical performance and patient reception for providers.
- Conduct annual/periodic performance reviews with each member of your team to assess and provide feedback on clinical performance, community relations, and other key performance areas.
- Various administrative/management tasks including, but not limited to, approving PTO/leave, payroll, coverage coordination, as assigned.
- Minimum of 1 year of experience with Bloom Healthcare working as a Nurse Practitioner, Physician Assistant or Physician.
- Minimum of one successful cycle as a new provider coach or student precepting.
- Good standing with the organization with no remedial actions.
- Must work a minimum of 80% of full-time hours.
- Strong leadership, communication, and organizational skills.
- Ability to work independently and as part of a team.
- Consistent standing of 80%% of note closure rate, <48h.
- Must consistently adhere to patient visit standards.
The salary for this position is competitive and commensurate with experience.
