MISSION:

POSITION SUMMARY:

Under the direct supervision of the Clinical Supervisor, this position is responsible for the management of psychosocial aspects of chronic and acute diseases. The application of behavioral principles to address lifestyle and health risk issues. Emphasis is placed on prevention and self-help approaches, partnering with clients in a treatment approach to build resiliency and encourage personal responsibility for health. The Behavioral Health Consultant provides consultation and co-management in the treatment of mental disorders and psychosocial issues while working with a fully integrated, multidisciplinary team in the primary care clinic.

RESPONSIBILITIES:

  • Provide behavioral health services to clients as part of the continuum of care.
  • Provide intake, assessment and referral services, as necessary utilizing DSM-IV/ICD-9 diagnostics.
  • Provide quality clinical consultation to providers and staff regarding mental and behavioral health issues.
  • Provide culturally sensitive, brief, evidence based, solution-focused behavioral interventions with active development of behavioral change planning.
  • Create education, administration, and self-management skills for targeted populations with psychological issues impacting physical and/or behavioral health issues.
  • Submit timely documentation into EHR of all services provided through client records and data (i.e., demographic data, treatment plans, psychosocial history, progress notes, discharge planning, etc.).
  • Promote knowledge growth among clinic staff regarding behavioral health issues and provide education to enhance care provided across all disciplines, thereby enhancing treatment provided.
  • Schedule and facilitate and/or actively participate in ART/CFT meetings as well as initiate clinical discharge planning from the beginning of services.
  • Maintain productivity standards and meet documentation deadlines.
  • Provide reports as requested by supervisor.
  • Complete coordination of care with referral sources and integrated health providers on clients’ progress and needs in treatment.
  • May supervise Interns as needed under the oversight of the Clinical Supervisor or the Clinical Director.
  • Support and actively engage in practices of inclusion through Cultural and Linguistically Appropriate Services (CLAS) Standards.
  • Other duties as assigned.

EDUCATIONAL/WORK REQUIREMENT:

  • Master’s Degree in Behavioral Health field
  • Independent licensure required (LCSW or LAC)
    • LIAC with LCSW or LAC only
  • Tier I $77,000 – $80,000
    • Up to 2 years of experience in mental health and/or substance abuse treatment.
  • Tier II $81,000 - $85,000
    • 3 – 5 years of experience in mental health and/or substance abuse treatment, or
    • 1 - 2 years of experience in similar role in an FQHC setting.
  • Tier III $86,000 - $91,000
    • More than 5 years of experience in mental health and/or substance abuse treatment, or
    • More than 3 years of experience in a similar role in an FQHC setting.

SKILLS REQUIREMENT:

  • MS Office proficiency.
  • Strong interpersonal skills with an ability to work effectively with a wide range of people, teams, managers, supervisors, and vendors.
  • Demonstrated working knowledge of the American Society of Addiction Medicine’s Patient Placement Criteria.
  • Familiar with DSM (most current version), ICD-9 and diagnostic criteria.
  • Demonstrate excellent written and oral communication skills.
  • Excellent customer service skills.
  • Experience in working with the Native American population preferred.
  • Must demonstrate critical thinking, problem-solving and organizational and time management skills.
  • Must be able to work well with others in a team approach.
  • Strong clinical skills required.
  • Possess a background in the delivery of individual/group/family counseling services, with a substance use population.
  • Fingerprint Certificate required.
  • Bilingual - Spanish preferred.

PHYSICAL DEMANDS:

  • Able to sit, stand and walk with/without accommodation.

FAIR LABOR STANDARDS ACT:

This position is considered to be Exempt for overtime pay provisions as provided by the Federal Fair Labor Standards Act (FLSA) and any applicable state laws. Non-Exempt employees are entitled to overtime pay for hours worked in excess of forty (40) hours per workweek.
