We are seeking a Mobile Health Provider (APRN or Physician Associate) who thrives in dynamic environments and is passionate about meeting patients where they are.

  • Deliver primary and preventative care outside traditional settings

🩺 What You’ll Do
  • Provide comprehensive primary care, including preventative exams, diagnosis, treatment, and follow-up
  • Coordinate care with internal teams and community-based resources (behavioral health, social services, specialty care, etc.)
  • Document all patient care accurately and timely in the EHR system
  • Participate in case conferences, provider meetings, and quality improvement initiatives
  • Support and mentor students (APRN/PA) when applicable
  • Contribute to the development and refinement of clinical protocols
  • Deliver care with professionalism, compassion, and respect

🧠 What You BringRequired Qualifications:

APRN:

  • Current NH RN & APRN license
  • Board Certified (ANCC or AANP)

OR Physician Associate:

  • NH Board of Medicine license
  • NCCPA Certification
  • Current BLS (ACLS/PALS preferred)
  • Current DEA registration
