Client Manager, Customized Client Care

This position is listed on behalf of a partner company, who manages all applications and next steps. Our partner is looking for a Client Manager, Customized Client Care based in United States.

This role provides comprehensive, compassionate, and client-centered support to individuals navigating complex health and financial challenges.
You will conduct assessments, support grant requests, and connect eligible clients with appropriate resources and services.
The position takes a holistic approach, recognizing the interconnected impact of financial hardship, physical health, and mental health.
You will apply trauma-informed and strength-based approaches while using clinical judgment to guide clients toward meaningful support.
The role combines direct client care, case coordination, data-informed program improvement, and cross-functional collaboration.
You will contribute insights that help strengthen services and respond to emerging needs within the music community.

Accountabilities:
  • Deliver equitable, coordinated, and client-centered care that considers the interconnected effects of financial hardship, physical health, and mental health on overall wellbeing.
  • Use trauma-informed and strength-based approaches to provide informal counseling, guidance, and client support within the scope of the role.
  • Apply clinical judgment to assess and recommend grant assistance for health, mental health, addiction recovery, and basic living needs while balancing grant administration with holistic support.
  • Connect individuals with appropriate internal pathways, community resources, and referrals to ensure access to a coordinated continuum of care.
  • Use client insights and data to continuously strengthen program and service delivery.
  • Maintain familiarity with annual Wellness in Music survey data and use emerging insights to better understand the needs of the music community.
  • Translate survey findings and client referrals into informed care coordination while identifying opportunities to strengthen programs and broader support strategies.
  • Apply consistent assessment tools and program guidelines while maintaining accurate, timely, and confidential case records in accordance with organizational policies.
  • Collaborate with Operations and Finance teams on documentation standards, fraud mitigation, and consistent approaches to client assessment.
  • Participate in ongoing professional development covering data, trauma-informed care, motivational interviewing, client engagement, industry trends, and Health & Human Services compliance.
  • Identify and communicate client-focused trends while maintaining a strong understanding of program stewardship and responsible resource management.
  • Collaborate across departments to support effective program delivery and client outcomes.
  • Represent the organization at onsite events and industry engagement opportunities while promoting available programs and services to relevant communities and affiliates.
Requirements
  • Master’s degree in mental health, behavioral health, social services, or a related field.
  • 3+ years of professional experience in mental health, behavioral health, social services, or a related environment.
  • Ability to assess and respond to a broad range of needs involving mental health, addiction recovery, health-related issues, financial hardship, and basic living needs.
  • Strong commitment to confidentiality, privacy, and responsible handling of sensitive client information.
  • Cultural competence and high emotional intelligence, with the ability to engage respectfully and effectively with diverse communities.
  • Strong proficiency in Microsoft Office, Mailchimp, Eventbrite, and/or comparable software and creative tools.
  • Knowledge of database systems and the ability to optimize data for reporting and departmental analytics.
  • Demonstrated leadership capabilities with excellent interpersonal, written, and verbal communication skills.
  • Strong strategic thinking, analytical abilities, and sound professional judgment.
  • Ability to build trusting relationships and communicate with empathy and professionalism.
  • Ability to thrive in a collaborative, entrepreneurial, and mission-driven environment.
  • Adaptability, initiative, and enthusiasm for working in a dynamic organization with a strong public-facing mission.
Benefits
  • Starting salary of $83,800–$93,000 per year, benchmarked against industry standards.
  • Actual compensation may vary based on experience, qualifications, geographic labor market, and role requirements.
