What You'll Do (Your Role & Impact)

• Deliver Compassionate Care: Carry out personalized plans of care directed by physicians to keep patients safe and comfortable at home.
• Prevent Re-Hospitalizations: Actively monitor condition changes to adjust care plans early and minimize hospital visits.
• Collaborate on Care: Communicate directly with physicians regarding medication needs and ongoing care adjustments.
• Empower Families: Teach and counsel patients and family members on managing injuries, diseases, and recovery processes.
• Digital Documentation: Complete patient care records efficiently in our WellSky EMR system.
• Continuous Growth: Attend orientation, ongoing staff meetings, and earn CEUs.

What We’re Looking For

• Active Michigan LPN License (in good standing)
• Current CPR Certification
• Reliable Transportation & current auto insurance
• Tech Comfort: Solid computer skills and ability to navigate an EMR system (WellSky experience is a plus!)
• Industry Knowledge: Working understanding of Medicare/Medicaid guidelines for Home Health
• Key Traits: Strong independent work ethic, a compassionate heart, and a passion for working with homebound patients
