Position Overview:
We are currently hiring full-time, part-time, and PRN Licensed Practical Nurses (LPNs) to join our home health care team. As a key member of our care team, you will deliver skilled nursing services directly in patients’ homes, helping them maintain independence and quality of life.

Responsibilities:

• Conduct comprehensive nursing assessments in patients’ homes to support individualized care plans.

• Administer skilled nursing procedures and treatments such as wound care, medication administration, injections, catheter care, and vital sign monitoring.

• Monitor patient progress and promptly communicate concerns to the supervising RN or provider.

• Educate patients and caregivers on disease management, medications, and self-care to promote independence.

• Provide compassionate emotional support to patients and families, especially during challenging health situations.

• Collaborate with physicians, therapists, social workers, and home health aides for coordinated care.

• Maintain accurate documentation in electronic health records (EHR).

• Support safe care environments by adhering to infection control and emergency protocols.

• Participate in team meetings, case conferences, and quality improvement initiatives.

Qualifications:

• Graduate of an accredited practical/vocational nursing program.

• Current LPN/LVN license in the state of employment.

• 2–3 years of nursing experience (home health or community health preferred).

• Strong clinical assessment and critical thinking skills.

• Excellent communication and patient education skills.

• Proficiency with electronic health records (EHR) systems.

• Valid driver’s license, reliable transportation, and willingness to travel within Prince George’s and Montgomery counties.
