About the Northwest Portland Area Indian Health Board (NPAIHB)
The Northwest Tribes have long recognized the need to exercise control over the design and development of health care delivery systems in their local communities. To this end, they formed the Northwest Portland Area Indian Health Board (also referred to as NPAIHB or “the Board”) in 1972.
Guided by a vision of “Health and Wellness for the Seventh Generation,” NPAIHB is a 501(c)3 designated (non-profit) organization and Tribal organization, P.L. 93-638, under the Indian Self-Determination and Education Assistance Act (ISDEAA) serving the 43 federally recognized Tribes in the states of Idaho, Oregon, and Washington (Northwest Tribes). Tribes become voting members of the Board through resolutions passed by their governing body. Each member Tribe designates a delegate to serve on the NPAIHB Board of Directors.
Led by our Board of Directors, NPAIHB’s mission is to “eliminate health disparities and improve the quality of life of American Indians and Alaska Natives by supporting Northwest Tribes in their delivery of culturally appropriate, high-quality health programs and services.” We have a staff of over 120 professionals dedicated to advancing Tribal health for the 7th generation in the Pacific Northwest.

Job Title: Chronic Disease Prevention Projects Director
Reports to: Clinical Support Services Division Director

Position SummaryThe Chronic Disease Prevention Projects Director (PD) leads, plans, and directs the goals and objectives for the Building Our Largest Dementia (BOLD) infrastructure grant, and supervises, guides, and trains project staff members to complete all aspects of the Northwest Tribal Elders Project (NTEP) planning, implementation, documentation and reporting (jointly referred to as the Chronic Disease Prevention projects). The PD is responsible for sustaining and expanding the projects’ scope of work by identifying additional funding opportunities, providing leadership in developing proposals, managing project budgets, writing mid-year and annual reports for all funding agencies, establishing project protocols for staff, disseminating project information to local and national groups and agencies, performing all administrative duties regarding project personnel, and assuring that the proposed goals and objectives of the BOLD program are fulfilled.
The PD is accountable for both the near-term and long-term outcomes as outlined in the project description. The PD meets on a regular basis with Finance and Grants staff regarding the budget and fiscal matters (such as the monthly finance report provided by Finance to the PD). The PD also attends regular NPAIHB Project Director meetings to discuss policy, budget, personnel, and Northwest Tribal priorities. The PD develops and implements a tribal strategic plan that outlines the goals and strategies of the Chronic Disease Prevention projects in collaboration with the Tribal Elder Advisory Committee and the Northwest Tribal Dementia Coalition. The PD develops and implements the community needs assessments to identify strengths and gaps in current programming available from tribal health and elder wellness programs, and identifies, adapts, and implements programs to support Dementia-related diseases.
The PD must have experience working with tribal communities, project management, and building partnerships with key stakeholders and tribal governments. The PD directs the Chronic Disease Prevention project teams of public health professionals who assist the PD with day-to-day administration to meet deliverables and implementation of the yearly work plans. The PD reports to the Clinical Support Services Division Director. This scope of work is housed within the Clinical Division. This position is based at NPAIHB’s offices in Portland, Oregon. Local travel and/or overnight travel outside of the area is required approximately 15% of the time.
Essential FunctionsBOLD & NTEP Program Excellence and Success· Serves as the primary contact for NTEP/BOLD Network inquiries and communication (promotional material requests, training & presentation requests, etc.)
· Provides BOLD and NTEP program direction and support in alignment with NPAIHB Strategic Plan
· Directs the delivery of the Chronic Disease Prevention projects which improve the quality of life for tribal Elders and older adults living with dementia, mild cognitive impairment, and traumatic brain injuries for both the Clinical Division and for other NPAIHB programs (including the THRIVE, TOR, TCHPP, ECHO, Cancer, and Tobacco programs), as well as the CDC, IHS ETHICS II, IHS CHAP, and SAMSHA grants, along with future projects and services
· Participates in (and leads, where appropriate) all NPAIHB Chronic Disease Prevention projects related grant meetings and program discussions
· Leads the accomplishment of the BOLD and NTEP program goals in collaboration with NPAIHB staff, Tribal partners, and consultants, including:
o Reducing the risk factors for Alzheimer’s disease and related dementias (ADRD)
o Improving the integration of caregivers into processes and systems
o Increasing awareness and understanding of ADRD topics among both the general public and by furthering professional development of providers
· Plans, coordinates, and implements the dissemination of information about ADRD risk reduction, early diagnosis, management, and the role of caregiving by scheduling and coordinating trainings on ADRD primary, secondary, and tertiary prevention to tribal communities and tribal health care providers and healthcare systems.
· Facilitates and provides staff support to the Elders Committee meeting during the Quarterly NPAIHB Board of Directors Meeting (QBM)
· Coordinates, prepares and assists NW Tribal Elder program staff with planning meetings, trainings, conferences and workshops
· Conducts long-term and short-term research (including surveys, feasibility studies, and research studies) to guide Chronic Disease Prevention projects development, improvement of services, and expansion of Tribally based best practices for NW Tribes
· Updates BOLD and NTEP program priorities throughout the year, as directed by Northwest Tribes, the NPAIHB Elders Committee, and through the NPAIHB Strategic Planning process and/or annual federal and state policy priorities
· Works closely with supervisor and other NPAIHB leadership staff to support the identification of Northwest, federal, and state policy priorities related to Chronic Disease Prevention projects
· Prepare NTEP/BOLD training materials (including PowerPoint presentations, announcements, manuals, handouts, etc.) and present at tribal and urban site visits and at regional & national trainings
NPAIHB Project Director Functions· Provides general support to during the QBM as advised
· Represents projects and NPAIHB at regional and national meetings and conferences
· Creates and implements capacity building and quality improvement strategies based on the needs and priorities of Northwest Tribes
· Oversees sustainability initiative for funding sources, including writing grants, finding funds, managing overall programmatic budgets every fiscal year, and modifying budgets as appropriate
Supervisory Responsibilities· Supervises one (1) project staff member (the BOLD Project Coordinator), including assisting with hiring, preparing performance evaluations, reviewing work plans, and assigning and reviewing new projects
· Prepares and manages contracts and workload with outside consultants
· Evaluates and coaches the performance and expectations of all staff and contractors, including identifying and coordinating training and professional development needs
· Ensures that staff and contractors are following organizational policies, procedures, and directives, and requirements of NPAIHB partnering agencies and other relevant parties
· Completes staff performance evaluations within 30 days of employee’s anniversary date, and within 30 days of the completion of employee’s six-month probationary period (as applicable)
· Works in collaboration with Executive Director and Human Resources on staff disciplinary procedures as outlined in the Program Operations Manual
Administrative and Reporting Functions· Manages the preparation and submission of all required project reports within the BOLD program
· Drafts and reviews project-related external contracts
· Engages staff in the collection of pertinent data to meet reporting requirements
· Administers budget expenditures through partnership with staff to ensure projects remain on budget and in compliance with the funding requirements of granting agencies
· Maintains well-organized filing systems for all the BOLD program related required documentation and supporting records (both electronic and hard copy) in compliance with both NPAIHB and funding agency requirements, including securely archiving files as appropriate
· Prepares activity reports for NPAIHB Delegates (for Quarterly and ad hoc Board Meetings), the clinical division director, eMar, and funding agencies
· Submits a Monthly Activity Report (MAR) to the Division Director of Clinical Support Servies at the end of each month, and any additional reports required to track project activities as needed

QualificationsEducationMinimum education required*:
· Bachelors degree in Public Health, Healthcare Administration, or related field
· *A minimum of 10 years of demonstrated experience in program management, grant management, or tribal relations may serve as an acceptable substitute
Additional education preferred:
· Master’s degree in Public Health, Healthcare Administration or related field
ExperienceMinimum experience required:
· At least 5 years of professional experience working with Tribes, Tribal organizations, Tribal communities, or Tribal health clinics
· At least 5 years of professional experience working in public health, tribal health, or tribal relations
· Direct experience as a mentor, and orientation toward mentorship
· Preparing written reports
Additional experience preferred:
· Direct experience working with Northwest Tribes
· Experience in the planning, organization, implementation, and follow-up of public health-related coalitions
Required Knowledge, Skills, and Abilities· Demonstrated knowledge, discretion, tact, judgment, and overall ability in working effectively with federal, Tribal, and other professionals, and facilitating participation and partnership in the activities of the BOLD Program within the Clinical Division of the NPAIHB

Travel RequirementsLocal travel is occasionally required (approximately 15% of the time). Overnight travel outside of the area is infrequently required (approximately 10% of the time). Attendance at quarterly NPAIHB Board Meetings is required. These meetings are held both locally and hosted by our member tribes on a rotating basis, and typically take place during the third or fourth weeks of January, April, July, and October, and require a minimum stay of two/three overnights per meeting.
