Health Information Management (HIM) Document Router II
- Full-Time
- Monday - Friday, day schedule
- Hybrid (2 days in office, 3 days from home)
- $17.75 - $26.00 per hour

Hybrid schedule: 2 days in office, 3 days from home
**Six-month minimum in office training and satisfactorily complete all essential job functions prior to hybrid**

The Health Information Management (HIM) Document Router II position routes and indexes patient documentation and incoming medical documents that are received by fax, e-fax, mail, and/ or secure direct message accurately and efficiently within the patient’s electronic medical record. Responsible for tracking open lab orders to ensure missing results from outside laboratories are received, lab orders are closed, and/or declined when ordered lab work is not completed.
Health Information Management (HIM) Document Router II responsibilities:
- Index and route a variety of e-faxed and/or direct secure message medical documents accurately into patient electronic health records in accordance with standard operating procedures to ensure document holding tanks are processed daily and remain free of backlog.
- Sort, retrieve, scan, and index a variety of paper medical reports into patient electronic health record.
- Review outstanding open lab reports each month, obtain missing results from HealtheConnections or Community Chart as needed, generate and send declined lab letters to patients when ordered testing is not completed, and update the lab status to declined.
- Link appropriate specialist visit notes in the patient's electronic health record to open referrals and ensure the referral gets closed.
- Parse specific lab results received via CCD or CCDA into electronic health records.
- Customize the Disease Maintenance and Health Maintenance formula for colonoscopy within the patient's electronic health record.
- Contact skilled nursing facilities, as needed, to obtain admission notes and discharge summaries requested by CNY Family Care Transition of Care nurses; ensure all records are received and uploaded to the patient’s electronic health record prior to the transition-of-care visit.
- Review outgoing medical record requests for completeness and ensure they are HIPAA valid authorizations.
- Perform regular quality checks to ensure all scanned documents are indexed according to standard operating procedures.
Health Information Management (HIM) Document Router II qualifications:
- High school diploma or general education degree (GED) required ; one to two years of college preferred
- 2 years’ experience working in a health care setting required. Experience in medical records very helpful.
- Knowledge and experience with Electronic Health Record required.
- Medent EHR experience candidates highly prioritized.
- Certification as an RHIT or in the process of obtaining is preferred.
