This position is listed on behalf of a partner company, who manages all applications and next steps. Our partner is looking for a Bilingual Care Manager, Telephonic Nurse based in United States.
This remote nursing role focuses on helping members achieve and maintain their best possible health outcomes through personalized, telephonic care management. You will assess individual needs, identify barriers to effective care, and connect members and families with appropriate clinical, community, and social resources. The position combines clinical expertise, critical thinking, health coaching, and care coordination in a member-focused environment. You will continuously monitor progress, develop and adjust care plans, and help guide members toward meaningful health outcomes. As a bilingual professional, you will communicate confidently in both English and Spanish while supporting members with diverse needs. The role offers experienced nurses an opportunity to apply their clinical knowledge beyond traditional bedside care while making a measurable impact on the health and wellbeing of the populations they serve.
- Assess and evaluate members' physical, environmental, and psychosocial health needs through telephonic interactions, clinical data, and ongoing conversations.
- Develop and implement individualized care plans designed to help members achieve or maintain optimal health and wellness outcomes.
- Monitor member progress toward desired outcomes and adjust care strategies based on assessments, changing needs, and available resources.
- Identify barriers that may prevent effective care and develop practical strategies to address or resolve those challenges.
- Guide members and families toward appropriate healthcare, community, social service, and wellness resources.
- Apply clinical judgment, critical thinking, and evidence-based approaches when determining appropriate courses of action.
- Coordinate care across multiple systems and resources while maintaining accurate and effective documentation.
- Use health promotion, coaching, education, and wellness strategies to encourage members to actively participate in their care.
- Communicate effectively in both English and Spanish when assessing needs, providing education, and coordinating services.
- Navigate multiple technology systems and information sources while maintaining confidentiality and protecting sensitive member information.
- Current Registered Nurse (RN) licensure in Illinois with no disciplinary action, or the ability to obtain Illinois RN licensure.
- At least 3 years of clinical acute care experience as a licensed RN.
- Bachelor's degree in a health-related field.
- Full professional fluency in both English and Spanish, including the ability to speak, read, and write both languages without limitations or assistance.
- Ability to successfully complete an approved language proficiency assessment for professional communication in Spanish.
- Strong clinical assessment, critical thinking, problem-solving, communication, and care coordination skills.
- Ability to work independently, interpret complex information, and determine appropriate courses of action in a telephonic care environment.
- Experience with case management, discharge planning, and patient education for adult acute care is preferred.
- Experience in managed care, Medicare and Medicaid populations, health promotion, coaching, or wellness is an advantage.
- Certified Case Manager (CCM) credential is preferred.
- Knowledge of community health organizations, social service agencies, and other community-based resources is beneficial.
- Ability to work effectively in a remote environment using multiple systems while maintaining a dedicated workspace and protecting confidential health information.
- Reliable home internet connection with at least 25 Mbps download and 10 Mbps upload speeds.
- Availability to work Monday through Friday, generally from 8:00 a.m. to 5:00 p.m. Central Time, with occasional scheduling flexibility based on member needs.
- Willingness to travel occasionally for training or meetings when required.
- Annual base salary range of $71,100–$97,800, depending on geographic location, qualifications, experience, education, certifications, and job-related skills.
