Job Description
Community Medical Group is seeking a friendly, dependable, and service-driven Social Services Agent to join our team.
In this role, you will help clients, visitors, and patients apply for government assistance programs, including Food Stamps (SNAP), Medicaid, Temporary Cash Assistance, and the Medicare Savings Program. You will guide individuals through the ACCESS application process, provide referrals to community resources, and help patients connect with healthcare coverage and services.

Compensation and Schedule• Competitive hourly pay (based on experience)

• Assist clients with completing and submitting ACCESS applications for Food Stamps (SNAP), Medicaid, Temporary Cash Assistance, and the Medicare Savings Program
• Explain the ACCESS application process and the role of the Social Services Agent versus the Department of Children and Families (DCF)
• Complete required DCF and Community Medical Group electronic consent forms and obtain client signatures
• Assist clients with creating and managing their MyACCESS accounts
• Monitor application status daily and follow up until eligibility is determined by the Office of Economic Self-Sufficiency
• Notify clients of eligibility determinations and explain next steps for approved benefits
• Educate Medicaid-eligible clients on their health plan options and assist with selecting Community Medical Group as their primary care provider when requested
• Assist with health plan enrollment and related documentation
• Document client demographics and all application activities accurately in company systems
• Provide referrals to community agencies and resources when appropriate
• Assist visitors with using courtesy phone services as needed
• Handle confidential information professionally while maintaining HIPAA compliance
• Ensure all information provided by clients is documented accurately without leading or influencing responses
• Perform other duties as assigned
• High school diploma or equivalent
• Previous customer service experience required
• Experience with DCF, ACCESS, Medicaid, SNAP, or other government assistance programs preferred
• Data entry experience and proficiency with Microsoft Office and client management software
• Strong communication and interpersonal skills
• Excellent organizational skills and attention to detail
• Ability to work independently while managing multiple priorities
• Strong problem-solving and decision-making abilities
• Ability to maintain professionalism and build positive relationships with clients and coworkers
• Understanding of confidentiality requirements and HIPAA compliance
• Bilingual in English and Spanish or English and Creole preferred
