Description:

About TRIARQ Health

At TRIARQ Health, we help independent physician practices succeed in value-based care, improving outcomes, lowering costs, and keeping physicians independent.

We build High-Value Referral Networks that connect specialists and primary care, enabling smarter referrals, stronger care coordination, and measurable performance improvement.

Within that network, Urology is one of our highest strategic priorities, a significant and growing driver of healthcare utilization and spend. By aligning urologists and primary care providers, TRIARQ ensures patients follow conservative, evidence-based care pathways while avoiding unnecessary procedures and costs.

We are seeking a Director, Provider Partnerships to set the strategic direction for our urology provider network and lead a team of practice partnership managers. This role combines business development, healthcare management, and provider partnerships expertise with executive-level relationship management, program strategy, and cross-functional leadership.

Now is an especially exciting time to join! TRIARQ is rolling out value-based programs with a large Michigan payer to transform how specialists and PCPs work together in urological care statewide, and this leader will shape that program's provider strategy from the ground up.

Role Overview

As Director, you will:

• Own the overall urology provider partnership strategy, setting priorities, targets, and playbooks that regional teams execute against.

• Build and lead a team of provider partnership managers, providing coaching, performance management, and professional development.

• Serve as the senior clinical and strategic advisor to urology practice leadership, bringing clinical credibility to conversations about care pathways and utilization.

• Own provider scorecard strategy and executive-level reporting, translating program data into strategic recommendations for TRIARQ and payer leadership.

• Set the standard for practice staff engagement, ensuring teams drive adoption of TRIARQ's care management and referral platforms across the urology network.

• Partner with member experience, care management, and analytics leadership to align provider engagement strategy with care pathway development.

• Serve as the executive voice for urology providers, representing their needs in TRIARQ's specialty care strategy and payer discussions.

This is a Michigan-based, field-facing leadership role where you'll travel to meet with practices, largely concentrated in the state's key geographic centers — mostly day trips with limited overnight travel — set the direction for urology provider adoption, strengthen referral networks statewide, and be accountable for the program's measurable success.

Key Responsibilities

• Lead the onboarding strategy for urology providers into TRIARQ's High-Value Referral Network and value-based programs, and oversee execution across the team.

• Design and oversee training curricula for physicians and staff on referral tools, care pathways, and performance dashboards.

• Lead executive-level quarterly business reviews with key practices and payer partners, driving accountability and program improvement.

• Ensure consistent, network-wide use of Care Management and Referral Management technology to strengthen in-network referrals.

• Partner with marketing, care management, and member experience leadership to promote engagement and conservative care pathways at scale.

• Own adoption metrics and program reporting, setting goals and holding the team accountable to provider alignment targets.

• Manage and develop a team of provider partnership managers, including hiring, coaching, and performance reviews.

• Represent the urology program in leadership and payer forums, contributing to program strategy and growth planning.

Requirements:

• 3+ years of experience in provider engagement or practice management, including experience in a value-based care setting.

• Experience in business development, healthcare management, or payer-facing program accountability.

• Strong executive communicator, able to translate complex clinical and performance data into strategic, provider-friendly insights for senior stakeholders.

• Advanced proficiency with EHRs, CRM, or care management platforms.

• Deep clinical and operational understanding of urology, men's health, women's health, or specialty practice operations preferred.

• Nurse Practitioner (NP) or Registered Nurse (RN) with urology or related specialty clinical experience preferred.
