About the Role

This is a field-based role in Albuquerque where you’ll provide home- and community-based care, working with a collaborative interdisciplinary team while maintaining a high level of autonomy in your daily practice. Key Responsibilities
- Conduct comprehensive palliative care assessments addressing physical, psychosocial, and spiritual needs
- Manage complex symptoms such as pain, dyspnea, nausea, and fatigue using evidence-based approaches
- Lead goals-of-care conversations, including advance care planning and end-of-life decision support
- Collaborate closely with physicians, social workers, chaplains, and other team members to deliver coordinated care
- Evaluate and facilitate appropriate transitions to home health or hospice services
- Document care plans and clinical decisions accurately and in compliance with regulatory standards
What We’re Looking For
- Active, unrestricted Nurse Practitioner license in New Mexico
- MSN or DNP from an accredited program; board certification (FNP, AGPCNP, or equivalent) required
- Prior hospice experience at some point in your nursing career
- Strong clinical judgment and confidence managing complex symptom burden
- Ability to navigate emotionally sensitive conversations with empathy and clarity
- Excellent communication and collaboration skills across interdisciplinary teams
- A patient-centered mindset and the ability to work independently with accountability
