Licensed Practical Nurse (LPN) at Comfort Plus Healthcare Services – Shorewood, Illinois

Location: Shorewood, IL
Pay Range: $35 – $42 per hour (up to $1500.00 + /weekly)
Schedule: Day Shifts – Tuesday to Thursday, 7:00 AM – 7:00 PM

• G-tube care
• Tracheostomy management

• Administer medications and treatments as directed by the supervising RN, ensuring safe and effective care for patients with complex medical needs.
• Monitor patient conditions continuously and report any significant changes to the RN in a timely manner.
• Document all patient interactions, treatments, and observations thoroughly and accurately in clinical notes.
• Maintain up-to-date and compliant clinical records, adhering to Comfort Plus Healthcare Services protocols and regulatory standards.
• Collaborate effectively with the care team to support each patient’s health maintenance and recovery goals.

• Active and unrestricted LPN license in the State of Illinois
• Prior experience in home health or a related care setting strongly preferred
• Strong clinical assessment, critical thinking, and documentation skills
• Excellent interpersonal and communication abilities to work with patients, families, and multidisciplinary teams
• Compassionate and patient-centered approach to care, with a focus on promoting independence and quality of life
