Description:

This is an opportunity to practice at the top of your APP scope while working remotely as an essential extension of a large, established cardiology practice. The Nocturnal APP plays a critical role in providing patients and healthcare partners with timely access to experienced cardiovascular clinical support when the daytime practice is closed.

The Nocturnal APP will serve as a centralized clinical resource for inpatient and outpatient cardiology calls, supporting patients, referring providers, hospital teams, and practice staff. This role requires strong clinical judgment, cardiology knowledge, and the ability to independently triage and manage urgent and non-urgent concerns.

Schedule

  • Remote/off-site nocturnal position
  • Overnight coverage
  • Nights, weekends, and holidays based on practice needs
  • Physician and subspecialty escalation available when clinically indicated

Key Responsibilities

  • Provide overnight clinical coverage through telephone and virtual communication.
  • Triage and manage inpatient and outpatient cardiology calls.
  • Assess clinical urgency and determine the appropriate level of care.
  • Address common cardiology concerns, including chest pain, arrhythmias, blood pressure concerns, heart failure symptoms, medication questions, anticoagulation, and post-procedure concerns.
  • Review medical records, laboratory results, diagnostic testing, medications, and other clinical information to support decision-making.
  • Collaborate with cardiologists, hospitalists, emergency departments, nursing teams, and other healthcare professionals.
  • Appropriately escalate emergent or complex clinical situations.
  • Provide patient education and reinforce treatment and follow-up plans.
  • Document patient and provider interactions accurately and timely in the electronic medical record.
  • Communicate relevant overnight events to the daytime care team to ensure continuity of care.

Preferred

  • Three or more years of cardiology or acute-care APP experience.
  • Experience with overnight, cross-cover, or remote clinical responsibilities.
  • Experience with inpatient cardiology or hospital-based care.
  • Knowledge of ECG interpretation, cardiac medications, anticoagulation, heart failure, arrhythmias, and cardiovascular emergencies.
Requirements:

Qualifications

Required

  • Graduate of an accredited Nurse Practitioner or Physician Assistant program.
  • Current, unrestricted Illinois NP or PA license.
  • Current national board certification.
  • Current BLS and ACLS certifications.
  • Clinical experience in cardiology, hospital medicine, emergency medicine, critical care, or another acute-care setting.
  • Strong clinical assessment, triage, communication, and decision-making skills.
  • Ability to work independently while recognizing when physician consultation or escalation is needed.
  • Comfortable providing care through telephone and remote communication platforms.
