The Physical Therapist plays a crucial role in the rehabilitation and recovery of patients in their home environments. The Physical Therapist will work closely with interdisciplinary teams to provide high-quality physical therapy services that address the unique needs of each patient, promoting functional independence and enhancing their overall well-being.The Physical Therapist is an integral part of the Home Health team. Under the direction of the Director of Nursing, the job responsibilities include the following:

Responsibilities:

• Patient Assessment: Conduct comprehensive physical assessments to determine the patient's condition, mobility limitations, and rehabilitation potential.
• Treatment Planning: Develop individualized care plans tailored to the specific needs and goals of each patient, in coordination with the patient's physician and interdisciplinary team.
• Physical Therapy Interventions: Administer skilled physical therapy treatments, exercises, and modalities to improve mobility, strength, balance, and pain management.
• Patient Education: Educate patients and their families on therapeutic exercises, proper body mechanics, and safety measures to promote independence and prevent injury.
• Progress Monitoring: Regularly evaluate and document patient progress, adjusting treatment plans as necessary to achieve desired outcomes.
• Communication: Maintain open and effective communication with the patient's physician, case manager, and other healthcare professionals involved in the patient's care.
• Documentation: Maintain accurate and timely documentation of patient assessments, treatments, and progress notes in compliance with regulatory and agency standards.
• Quality Assurance: Adhere to all state, federal, and agency regulations and guidelines to ensure the highest standard of care.
• Equipment Management: Ensure the proper use and maintenance of therapy equipment and assistive devices.
• Professional Development: Stay current with advances in physical therapy practices and participate in ongoing education and training opportunities.
• Ethical Standards: Uphold the highest ethical standards and professionalism in all interactions with patients, families, and colleagues.

Qualifications:

• Valid Physical Therapist license in the state of Texas.
• CPR certification.
• Previous experience in home health or a related healthcare setting is preferred but not required.
• Strong clinical and interpersonal skills.
• Excellent written and verbal communication skills.
• Ability to work independently and as part of a collaborative team.
• Reliable transportation and a valid driver's license.
• Ability to lift and carry equipment as needed.
