The Staff Pharmacist is responsible for ensuring the safe, effective, and appropriate use of medications for all patients. Working under the direction of the Director of Pharmacy, the pharmacist provides both clinical and operational pharmacy services, including patient rounding, medication reconciliation, and provider consultation. This position plays a key role in improving patient outcomes through direct patient education, collaboration with providers, and adherence to regulatory standards.

Clinical Services

• Provide patient counseling and education regarding prescribed medications.

• Dispense, verify, and compound medications in accordance with physician orders.

• Manage drug interaction, drug allergy, and adverse reaction alerts.

• Perform drug utilization reviews, provide drug information, and support antimicrobial stewardship activities.

• Participate in multidisciplinary rounding to review patient profiles, recommend dosage adjustments, and reconcile medications.

• Prepare and manage specialized therapies including IV solutions, continuous epidurals, parenteral nutrition, and chemotherapy.

Operational/Non-Clinical Functions

• Purchase, inventory, and manage pharmaceuticals using Omnicell and pharmacy software systems.

• Maintain accurate controlled substance records in compliance with federal and state regulations.

• Operate and maintain laminar-flow hoods and IV pumps.

• Generate billing reports and ensure proper documentation in patient records.

• Stay current with changes in technology, regulations, and pharmacy best practices.

Patient-Centered Rounding Functions

• Conduct thorough medication reconciliation upon admission.

• Consult with patients on their home medications and ensure accuracy of medication histories.

• Provide patient-friendly discharge medication lists and education.

• Identify and resolve potential drug interactions, allergies, or dosing discrepancies in collaboration with medical staff.

Additional Duties

• Respond to staffing needs, including working alternate shifts or on-call coverage (must report within 60 minutes of call).

• Support all age groups from neonatal to geriatric populations with safe and effective pharmaceutical care.

Education:

• Doctor of Pharmacy (PharmD)

• Current New York State License and Registration as a Pharmacist.

Experience:

• Minimum of one (1) year experience in a hospital pharmacy setting required.
