Medical Assistant/LPN: Case Manager: Starting Pay Range: $18.39 - $26.00 an hour

Job Summary:

A team member of nursing service, that participates in the delivery of health care to the patients of Pueblo Community Health Center.

Clinical Programs Nursing Manager

Supervision Exercised:

Education/Experience/License/Certification:

• A current Colorado license - Licensed Practical Nurse (L.P.N.) or Medical Assistant who graduated from an accredited program
• Current B.L.S. with skills demonstration C.P.R. certification for Healthcare Providers required
• One year as an L.P.N. or Medical Assistant
• Current certification or registration through NHA, AMT, AAMA or other recognized credentialing organization is preferred .
• Valid driver’s license and auto insurance as required by law

Required Travel:

• Occasional travel is required including day-to-day local travel

Essential Duties and Responsibilities:

1. Ensure the function and activities of this department to embrace the philosophy, mission, values and Communicate with Heart service model supported by the Board of Directors of Pueblo Community Health Center, Inc.

2. Adhere to the guidelines and procedures of Pueblo Community Health Center, Inc.

3. Knowledge of all case management programs (WWC, EIS, Colorectal Cancer, Geriatric, Adolescent and Pediatric Immunizations).

4. Case managers assigned to program(s) and duties within the programs per Clinical Programs Nursing Manager.

5. Knowledge of Next Gen scheduling, insurance, patient demographics, medical discharge, etc.

6. Knowledge of Next Gen EPM and EHR

7. Knowledge of CIIS and ECast

8. Knowledge of all Electronic Tracking Systems used within the Department.

9. Assist a Medical Provider in providing patient care. This will include the following, but not limited to:

• Provides the information to the patient concerning preparations for laboratory/radiographic examinations. Also will serve as an informational source to the patient concerning Physician Referrals, Medical Triage, and Medications prescribed by the Medical Provider.
• Utilize appropriate forms used in patient care in medical record; (Diabetic flow sheets, WCC, Past Medical History, Health Care Maintenance Depression Screen, etc.) during EMR outage
• Per medical providers order: may schedule laboratory, radiographic studies using appropriate documents per Nursing P &P.
• Administration of immunizations/medications as prescribed by the medical provider and document per nursing policy & procedure manual.

10. Adhere to all nursing and infection control policies and procedures.

11. Be a team member and participate in ongoing evaluations of systems

12. Provide nursing/medical assistant health care services appropriate to the age-specific needs of all patients in accordance with the principles of growth and development for infant, child, adolescent, adult and geriatric life cycles.

13. Maintain current licensure and CPR certification.

14. Perform other related duties as assigned by Clinical Programs Nursing Manager.
