Pediatric Licensed Practical Nurse (LPN) – Home Care

Location: All Five Boroughs of New York City (Bronx, Brooklyn, Manhattan, Queens, Staten Island)

LPN Wound Care Visits: $80.00 per case
Expected Hours: 16–40 hours per week

At Affirmed Home Care, we believe exceptional pediatric care begins at home. We are seeking compassionate, skilled, and dependable Licensed Practical Nurses (LPNs) to provide one-on-one care for pediatric patients throughout New York City's five boroughs.

• Provide individualized skilled nursing care to pediatric patients in their homes

• Administer medications and treatments as prescribed

• Monitor patient conditions and report significant changes promptly

• Manage specialized medical equipment, including:

  • Gastrostomy Tubes (G-Tubes)

  • Tracheostomies (Trachs)

  • Ventilators

• Collaborate with physicians, caregivers, and interdisciplinary healthcare teams

• Educate families on patient care procedures and safety measures

• Maintain accurate and timely clinical documentation in accordance with agency standards and regulatory requirements

• Current New York State Licensed Practical Nurse (LPN) License – Required

• Minimum of one (1) year of LPN experience – Required

• Pediatric nursing experience – Strongly Preferred

• Fluent in English (verbal and written communication)

• BLS Certification – Preferred

• PALS Certification – Preferred

• Fully vaccinated against COVID-19 – Preferred

• Authorized to work in the United States (ITIN not accepted)
