We are expanding and adding a Wellness Nurse Supervisor (LPN)

Position Summary:

The Licensed Practical Nurse (LPN) is responsible for performing selected nursing tasks in accordance with the Illinois Nurse Practice Act and under the supervision of a Registered Nurse (RN). The LPN will provide care to patients, administer treatments and medications, report changes in patient conditions, and maintain accurate clinical records.

Primary Responsibilities:

1. Monitoring and Reporting:

   • Monitor patients’ conditions and promptly report any changes to the supervising RN.

   • Administer treatments and medications as directed by the RN for the care of ill, or injured patients

   • Focus on health maintenance, illness prevention, and the development of a care plan based on the patient’s needs.

2. Client Intake Assessment and Patient Care:

   • Support health maintenance and illness prevention through patient care and education.

   • Perform nursing tasks within the scope of practice as defined by the Illinois Nurse Practice Act.

3. Clinical Recordkeeping:

   • Maintain accurate and detailed clinical records for each patient receiving care.

   • Ensure all observations and communications are accurately documented in clinical notes.

4. Communication with Physicians

   • Provide progress notes to the patient’s physician or podiatrist when the patient’s condition changes, when there are deviations from the plan of care, or as follows:

   • Every 60-90 days for home nursing agency patients.

5. Supervision of Home Health Aides:

   • Make home health aide assignments and prepare written instructions for their tasks.

   • Supervise home health aides in the patient’s home.

6. Medication and Treatment Administration:

   • Administer medications and treatments as prescribed by the patient’s physician or podiatrist.

7. Team Coordination:

   • Act as the coordinator of the healthcare team, ensuring proper communication and linkages across the continuum of care.

Physical Requirements:

1. Mobility and Strength:

   • Frequent standing, walking, bending, and reaching.

   • Ability to lift, push, pull, and carry up to 50 pounds independently.

   • Assist patients with transfers, positioning, and mobility.

2. Manual Dexterity:

   • Fine motor skills for operating medical equipment, administering medications, and documenting care.

3. Sensory Requirements:

   • Vision adequate to assess patient conditions, read labels, and complete documentation.

   • Hearing sufficiently to respond to patients, alarms, and team communications.

4. Stamina:

   • Ability to sustain physical activity for extended periods during patient care or emergencies.

   • Frequent travel between patient homes, requiring reliable transportation.
