Job Overview

The experienced Transplant Coordinator Registered Nurse for the Outpatient Clinic manages the comprehensive care continuum for pre-and post-transplant patients. This role acts as a primary clinical resource and multidisciplinary liaison. It ensures high-quality, patient-centered and strict regulatory compliance.

Qualifications

Required

  • Registered Nurse in the state of New Jersey
  • Bachelor's degree in nursing preferred
  • BLS via American Heart Association upon hire
  • Phlebotomy Certification
  • Three to five years of nursing experience required
  • Outpatient Clinic role must seek and maintain current certification for Basic Life Support via the American Heart Association, Phlebotomy and I.V. Therapy.
  • Completion of all orientation programs

Preferred

  • Certified Clinical Transplant Coordinator (CCTC) certification preferred
  • Experience in renal dialysis and/or transplantation preferred

Special Skills

  • Strong IV Skills required
  • Must seek and maintain in-depth knowledge of transplant nursing,
  • Excellent interpersonal skills,
  • Proficiency with computers,

Scheduling Requirements

  • Per Diem
  • Day Shift, Monday to Friday, 8-hour shift, 7.5 hours
  • Shift: 7:30AM to 3:30PM or 8AM to 4PM
  • No weekends or overnight
  • No major holidays
  • No on call required

Essential Functions

Coordinates nursing care and follow-up for post-transplant patients in the Transplant Clinic and Transplant Short Stay Unit settings. Manages, as assigned, the patient caseload by monitoring patient status, communicating changes in care plans, coordinating follow-up care, and addressing patient concerns through clinic visits and telephone triage.

Provides direct nursing care to patients in the Clinic and Short Stay Unit, including IV infusions, following established policies and procedures as assigned.

Review laboratory, diagnostic, and clinical data; identify significant findings; and collaborate with physicians and advanced practice providers to develop and implement appropriate treatment plans.

Conduct comprehensive post-transplant assessments, including physical, psychosocial, educational, and age-specific needs, and revise care plans based on patient progress as assigned.

Coordinate all aspects of transplant patient care, testing, procedures, admissions, follow-up appointments, and transitions across the continuum of care as assigned.

Provide patient and family education regarding transplantation, medications, treatment plans, self-management, discharge instructions, and long-term post-transplant care requirements as assigned.

Maintain accurate, timely, and compliant clinical documentation in the electronic medical record and transplant databases, including assessments, interventions, diagnoses, medications, and regulatory reporting requirements.

Facilitate referrals and collaboration with interdisciplinary resources, including social work, nutrition, financial services, and other support services to address patient and family needs.

Contribute to transplant research activities, regulatory compliance efforts, and departmental initiatives while maintaining required certifications and professional competencies.

Provide support within the transplant program by collaborating with managers, supervising support staff activities, assisting with problem-solving, and promoting efficient clinic and short-stay unit operations.
