Hospitalist Per Diem Advanced Practitioner in Schenectady, New York at Ellis Medicine

Ellis Medicine is seeking a Per Diem Hospital Medicine Advanced Practitioner to join our collaborative Hospital Medicine team. This is an excellent opportunity for an experienced APP or new graduate seeking flexible clinical work within a well-supported Hospital Medicine program.

Our APPs play an integral role in the care of hospitalized patients, working collaboratively with physicians while managing a dedicated patient census. With strong physician support, broad specialty availability, a closed ICU and no required procedures, our APPs can focus on providing high-quality, patient-centered care.

Clinical Responsibilities

Per diem Hospital Medicine APPs will:

• Provide comprehensive inpatient care in collaboration with Hospital Medicine physicians
• Evaluate and manage hospitalized patients
• Perform daily rounds and develop individualized treatment plans
• Complete admission and discharge evaluations as assigned
• Coordinate care with physicians, nursing staff and consulting specialists
• Communicate treatment plans and clinical updates with patients and families
• Participate in multidisciplinary care planning
• Coordinate transitions of care and follow-up needs
• Provide evidence-based, patient-centered care

Qualifications

• Current NP or PA certification
• Licensed (or eligible) to practice in New York State
• Hospital medicine or inpatient experience preferred but not required
• Strong clinical judgment and communication skills
• Ability to work collaboratively within a multidisciplinary team
• Commitment to high-quality, patient-centered care

Compensation

• $71.50/hour
• Opportunity to pick up additional shifts based on departmental needs and availability
